Description
The Centers for Medicare & Medicaid Services is seeking to award a fixed price commercial item contract, using simplified acquisition procedures for the printing of ?Medicare Publications In Braille?, under NAICS code 323119 titled ?Other Commercial Printing?. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number 402-5-371004. The solicitation is issued as a request for quotation (RFQ). Provision at 52.212-1, Instructions to Offerors?Commercial, applies to this acquisition. In accordance with the following Statement of Work: Statement of Work Statement of Work for a DHHS, Centers for Medicare & Medicaid Services (CMS) performance-based contract to purchase Braille transcription services and materials during a (base year 7/1/05-5/31/06) with one option year, July 1, 2006 through May 31, 2007. In addition to transcribing an average of 8 to 10 publications in the base year of this optional-year contract, purchased services will also include proofreading, packaging, and distribution of Braille publications. Task-order #1 for this contract is the production of an estimated 2,300 Braille copies of the Medicare & You 2006 handbook, which will begin in mid-July, 2005, and will terminate with the delivery of Braille handbooks by mid-September, 2005. Within seven business days of receiving written notice of contract award, the contractor will receive the following Government-Furnished Items (GFI) for task-order #1: a QUARK file of the M&Y 2006 handbook; a project plan template; a weekly project-update template; and an Excel file listing the ship-to names, addresses, and number of Braille handbooks ordered. Medicare publications are to be transcribed into grade II English Braille, on suitable paper stock, in compliance with Library of Congress National Library Services production standards. Braille publications that are two-volume sets are to be shrink-wrapped into a single package containing both volumes. Bulk shipment of Braille publications sent to the Cedar Rapids and Baltimore warehouse sites are to be packed five sets (10 Braille volumes) per carton for storage and secondary shipping purposes. Shipments of Braille publications sent to the CMS warehouse in Baltimore and the Pearson Fulfillment Center in Cedar Rapids, IA must be insured and sent by way of a traceable post/freight service. Samples of pre-printed mailing labels for the two fulfillment centers are included as part of the government-furnished items. Smaller shipments of Braille Medicare publications sent to addresses other than the CMS fulfillment centers should be packaged suitably and may be sent as ?free material for the Blind.? B.) Product Identification: Every Braille volume of a Medicare publication must display a black & white copy of the cover of the ink-print version of the publication. In addition, the CMS publication number and revision date must also be prominently displayed on the front cover of every Braille volume. Contractor Evaluation & Reporting: A.) Within 10 business days of receiving written notification of contract award, the Braille transcription contractor will submit a project plan to the CMS Project Officer for task-order #1, the Medicare & You 2005 handbook. See list of Government-Furnished Items (GFI) for a sample of a Medicare publication project plan. Project plans are required for all task-orders within the contract and may be submitted electronically to the CMS Project Officer for review and approval. B.) At the completion of each sub-task of a Braille publication production, the contractor will submit an electronic project update report, before proceeding to the next sub-task. See GFI listing for sample of project update template. Government-Furnished Items: A.) A QUARK file of the Medicare publication to be transcribed into Braille B.) An Excel file listing the ship-to names, addresses, and quantities ordered for two fulfillment centers and an estimated 50 to 75 non-beneficiary addresses C.) An electronic version of a CMS project plan D.) An electronic version of a CMS project-update plan E.) Sample address labels for bulk shipments of Braille publications to CMS fulfillment centers F.) A sample of a 2004 Braille edition of the Medicare & You handbook Contractor Deliverables: A.) A complete project plan must be submitted to CMS for review and approval prior to beginning the Braille production of any Medicare publication. Plans may be submitted electronically. B.) A project-update report is due to the CMS Project Officer at the completion of every sub-task listed in a project plan. C.) The final delivery of the complete order of finished products to all destinations must be completed by the agreed upon due-date. This contract includes information for task-order #1: the production of an estimated 2,300 Braille copies of the Medicare & You 2005 handbook. A tentative schedule of additional task-orders for Braille publications ordered during the base-year of this contract will be sent to the contractor no later than October 1, 2004.
