- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Army Community Service (ACS), Family Advocacy Program (FAP), Fort Carson, Colorado is seeking quotes for Parent Education & Support and Safety Education Services. Program is designed to nurture the military parent/child, which includes child abuse prevention, education and awareness services for parents, children, teachers, caregivers and all concerned adults in the community. Statement of Work - Contractor shall develop and manage progra ms: (1) To enhance the capabilities of parents to understand, nurture, communicate and relate more effectively with their pre-school age children, (2) To support parents of young children, reduce isolation and enhance outreach to young families. Activities may include a mothers support group, family life skills classes and special events, (3) To help the angry or spirited child, (4) To help children/youth protect themselves against neglect, child abuse and exploitation including child sexual abuse, (5) To improve parent/child communication, decision making, conflict resolution and self-esteem, (6) To educate children/youth enrolled in Child/Youth Service activities (six years and above) in local on and off post schools. As a minimum, contractor will provide the following FAP classes to soldiers and family members: (1) Nurturing 0-5 Parenting Programs, 3-6 iterations per year of the twelve-week program (consisting of 3-hour weekly sessions), (2) Nurturing 5-12 Parenting Programs, 3 iterations per year of the twelve-week program (consisting of 3-hour weekly sessions), (3) Baby Nurturing, weekly 1 ? hour classes for parents and their babies (0-15 months), (4) Playgroup, weekly 2-hour free play and child-centered activity group for parents and their pre-school children, (5) Infant Massage classes, weekly four-session (1 ? hour) classes to be offered every other month (about 6 times per year), (6) Nurturing PEACE, 3 iterations per year of twelve-week program (consisting of 3-hour weekly sessions) for parents and their children who have witnessed domestic violence, (7) Kids on the Block, puppet shows about child safety to be offered during year to elementary schools in districts 8, 3, 2 (plus additional districts upon request). Contractor shall maintain lesson plans and use materials provided by the ACS Family Advocacy Program. Examples of materials include Nurturing 0-5 Program, Nurturing 5-12 Program, Nurturing Baby, Nurturing PEACE, Teaching Infant Massage, Teaching and Leading Young Children, Childrens Dom estic Abuse Program, Dare To Be You, Baby Think It Over, Kids on the Block puppets/puppet show scripts. Coordination with the ACS Family Advocacy Program, New Parent Support Program, ACS Outreach Program, Social Work Service, Child & Youth Services staffs and ACS volunteers to provide programs/services is required. Customer service and evaluation forms from clients shall be used to determine use and customer satisfaction. A satisfaction rating of 85% or higher must be maintained. Contractor will devote up to 4100 hours annually (including evenings, early mornings and weekends) in delivering services. Performance will normally occur between 0730-1615, Monday-Friday. Services will not be provided on federally designated holidays or installation designated tra ining holidays. A minimum of 3 days notice will be provided before services are required. The services are to be performed at Fort Carson, Colorado and the surrounding community. Contractor shall compile weekly and monthly statistics to include class rost ers of clients served. All records and training materials will be property of the U.S. Army. The Government will provide space in which to conduct parenting and child safety education programs, office supplies, commercial class A telephone access, office equipment to include desk, chair, use of computer, training equipment such as projectors and VCR, graphic support to prepare training aids and publicity, and cop y reproduction services. Evaluation will be a two-step process. Those offers that are found to be technically acceptable will be evaluated for past performance and price, with past performance being somewhat more important than price. Contractors will firs t be determined as technically acceptable in accordance with both parts listed below. If determined to meet the minimum technical standard, contractors will then be evaluated on price and past performance. The first part of the technical standard includes all of the following minimum education and experience requirements for the primary service provider: (1) Earned masters degree from an accredited college or university in sociology, psychology, counseling, education, human services or other human resource field, (2) At least two years of experience teaching or working with adults and children/youth in a social service setting, (3) At least two years of experience providing educational/social services to clients in a military community, (4) At least 1 year of experience teaching or working in spouse and child abuse, (5) At least 2 years of experience with the Nurturing Parent Programs. Contractor is under no obligation to provide any specific employee, only that the primary service provider meet the minimum requirements. In order to establish continuity with both the serviced population and with the Government staff, the contractor must perform the bulk of the required services with one primary service provider. However, it is recognized that additional provi ders or substitutes may occasionally be necessary in order to perform all required services. The contractor must submit qualifications of additional service providers for Government approval before using them to perform under this contract. At its discreti on, the Government may terminate this contract for cause based on excessive use of substitutes. The second part of the technical standard is a description as to how the contractor intends to meet contract requirements. Request is for a future plan, not a l ist of past accomplishments. Description shall be sufficient such that the Government can be reasonably assured of the contractors ability to perform the service. Contractors shall provide pricing for 2 line items over a 5-year period. Line item 1 corresp onds to services outlined in above statement of work. Line item 2 corresponds to proficiency training in the related field. Training should consist of 1 conference/seminar annually or equivalent. Pricing shall be listed on an annual basis for one base year and 4 option years. Period of performance for base year is 14 July 2005 13 July 2006. Contractor proposals shall include the following: (1) Statement attesting to technical qualifications of service provider, (2) Statement as to how contractor intends t o fulfill requirements in statement of work, (3) Price quotations, and (4) No more than 3 past performance references. Government reserves right to check references not provided in proposal. Award shall be made only to contractors who have registered with the Central Contractor Registration at http://www.ccr.gov.
