- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. DESTCRIPTION: Southern Region Contracting Center West, Attn: SFCA-SR-W-M, Building 4197 2107 17th Street, Suite 15 Fort Sam Houston Texas 78234-5015 intends to procure ICE Clear 10 LB Block estimated usage is 20,000 blocks each and ICE Crushed, 20 LB Bags each estimated usage 10,000 Bags for a 12-month period. Contractor must provide delivery date. If the items are on a GSA schedule contract, provide GSA contract number, expiration date, terms and conditions. Request for quotation number W9124J-05-T-0083 due June 28 2005. The solicitation is for commercial items prepared IAW the format in FAR 12.6, as supplemented with additional information included in this notice. Contract period shall be from 1 Oct 2005 through 30 Sep 2006 with an additional two opti on years through 30 Sep 2008. The following clauses/provisions apply to this acquisition and are hereby incorporated by reference: FAR 52.212-1 Instructions to Offerors-Commercial, without addenda, applies to this acquisition. Offerors shall include comp lete copy of provisions at FAR 2.212-2 Evaluation commercial items evaluation factor ((i) meet qualifications of solicitation requirement: Prospective Offeror must be listed in the directory of Sanitarily Approved Food Establishment for Armed Forces Pro curement; (ii) availability/delivery time; (iii) past performance (see FAR 15.304); price.) Offerors shall include a complete copy of the provision at, FAR 52.212-3 Offeror Representation and Certification commercial items with offer. The provisions at FAR 52.212-4, Contract Terms and Conditions commercial items, with addenda, FAR 52.212-5 Contract Terms and Conditions addenda, apply to this acquisition and FAR 52-216-2, as amended by Contracting (52.222-35; 52-222-36; 52-222-37; 52.217-8 and 52.217-9 ). This procurement is set aside for small business under NAICS 312113 with a threshold of 500 employees; all responsible sources may submit an offer will be considered. Proposals may be provided on company letterhead stationery and shall provide this offi ce the following information: Request for Quotation Number, company name, address, telephone number, fax number, cage code, Duns number, tax identification number, point of contact, and correct mailing address.
