Description
This is a sole source combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitatio n; proposals are being requested and a written solicitation will not be issued. This non-competitive action will be processed in accordance with FAR 6.302-1, (Only one responsible source and no other supplies or services will satisfy agency requirements). L.M. Dulye and Company, 52 Kain Road, Warwick, NY 10990-0052 (a small woman-owned business) is the only known source that can provide the Phase III Implementation Phase of Communication Improvement Project. A sole source quote is being requested pursua nt to this announcement and shall reference Request for Quotation (RFQ) number W912EQ-05-T-0022. Following is a Statement of Work for this requirement. Statement of Work 1) Creation of the Implementation Team The contractor shall assist the Public Affairs Officer in the creation of Employee Implementation Team with cross-functional representation. The teams will be formed through a nomination process in late May, early June 2005, and then convened in June with the task of implementing the five recommendations approved by Colonel Smithers and his Directors to address the critical issues the Phase II research team revealed. 2) Scheduled Meetings The contractor shall facilitate approximately 17 implementation team meetings beginning on May 26th and ending in September. The tentative meeting schedule follows: (10 on-site visits by Linda Dulye and 7 teleconferences) May- 1 day on-site 37 hrs off-site 2 T/L June- 2 days on-site 25 hrs off-site 2 T/L July- 2 days on-site 25 hrs off-site 2 T/L Aug- 3 days on-site 25 hrs off-site 1 T/L Sept- 1 day on-site 50 hrs off-site 1 T/L 3) On-Site Consulting Services L.M. Dulye will work onsite to plan, develop, present and implement an execution strategy and plan. Onsite support will include strategic planning with management, facilitation of management meetings; planning sessions; executive coaching and mentoring; an d speaking engagements or presentations at management meetings. Onsite consulting also includes facilitation of employee teams and the use of L.M Dulyes proprietary project management, team facilitation and communication planning methodology and tools. L. M. Dulye will also provide coaching for team members, management and front-line supervisors on their roles and actions they can take to support and advance implementation. 4) Additional Services Performed from L.M. Dulye & Co. Offices These services will include teleconference facilitation of employee communications teams, management/leadership coaching, key message development, research, editorial services, review and response to project correspondence, data analysis, project managemen t, teleconferences for review, coaching and information exchange, and other communications support services. These services are performed at L.M. Dulye & Co. offices. This solicitation incorporates all of the applicable provisions and clauses in effect through FAC 05-04, as if stated in full text, as well as the terms and conditions of FAR 52.212-1, Instructions to Offerors  Commercial Items. Offerors should complete and include a copy of the provision 52.212-3 Alt I, Offeror Representations and Certifications-Commercial Items, with the offer. The following FAR clauses apply to this acquisition: Clause 52.212-4, Contract Terms and Conditions-Commercial Items; Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items; Clause 252.204-7004, Required Central Contractor Registration; Clause 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisition of Commercial Items; and 252.225-7002, Qualifying Country Sources as Subcont ractors. A Firm Fixed Priced Commercial Items Purchase Order, utilizing simplified acquisition procedures, will be used.
