- The Centers for Disease Control and Prevention intends to purchase on a sole source basis the following from Meta Communications, 1210 S. Gilbert St., Iowa City, IA 52240: Bus. Sol. Enterprise Base ?V Unlimited Users - Qty 1 Virtual Ticket/Job Manager 25 User License Pack- Qty 1 Virtual Ticket/Job Manager 50 User License Pack- Qty 1 Virtual Ticket/ Job Cost Plus 25 User License Pack- Qty 1 Virtual Ticket Production Pack (per seat license) - Qty 25 Virtual Ticket LZW Enabler (per server license)- Qty 1 Virtual Ticket Web Server Base ?V Runtime- Qty 1 Virtual Ticket Web Server Unlimited UL Pack- Qty 1 SQL SDK for Virtual Ticket/Job Manager- Qty 1 Bus. Solutions 1-yr. Software Assurance & Support- Qty 1 On-site Consulting & Training- Qty 1 day On-site Consulting & Training- Qty 1 additional day Meta Communications, is the only source known to the government that can provide the products and services with the above characteristics in accordance with the attached requirements. However, if any interested party believes it can meet the above requirements, it may submit a capability statement and price quotation. The capability statement, price quotation, and any other furnished information must be in writing and must contain material in sufficient detail to allow CDC to determine if the party can fully meet the requirements specified herein. The Government intends to evaluate offers and award a contract without discussions. Therefore, the initial offer should contain the offeror's best terms from a technical and price standpoint. Source Selection and Award will be made to the lowest price offeror whose proposal is rated technically acceptable. The North American Industry Classification (NAICS) Code is 541511 and the Size Standard is 500 employees for this requirement. All organizations may submit a price quotation for this RFQ. However, the Government will evaluate offers only from organizations that are certified by the U.S. Small Business Administration as small business concerns. If the Government's evaluation reveals that none of the small business concerns are technically acceptable, then the Government will evaluate all offers (regardless of their size status). Each offeror needs to identify and certify its size status in the Representations and Certifications submitted with the offer. Interested vendors capable of furnishing the Government with the services in this synopsis and Statement of Work attached should submit quotations according to this email and send it to the email address listed above. The requirement will be procured as a commercial item in accordance with FAR Part 12.102(c) under the test program. A determination by the Government not to compete this proposed requirement based on responses to this notice is solely within the discretion of the Government. The quotation shall include the itemized price, prompt payment terms, the F.O.B. Point (Destination or Origin), Taxpayer Identification Number (TIN), Dun & Bradstreet Number (DUNs), and descriptive literature of the product quoted. The quotation must reference Solicitation Number 2005-Q-02042. Quotations will be due on June 30, 2005 by 5pm Eastern Standard Time. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted via email to gmullins1@cdc.gov. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. The following provisions apply to this acquisition; 52.212-1 Instructions to Offerors - Commercials Items (JAN 2005); 52.212-2, Evaluation - Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical capability of the proposed system to meet the requirements stated herein, and (2) price. The clause 52.212-4, Contract Terms & Conditions - Commercial Items (OCT 2003) applies to this acquisition. The following FAR Clauses are hereby added to this clause: 52.247-35 F.O.B. Destination; The clause at 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (APR 2005) applies to this acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquisition: (1), (3), (5), (7), (9), (11), (12), (14), (15), (17), (18), (20) and (24).
