Description

This announcement is issued for informational and planning purposes only. This Request for Information (RFI) is not a Request for Proposals (RFP) and is not to be construed as a commitment by the Government to issue a solicitation or ultimately to award a contract on the basis of this RFI or otherwise pay for the information solicited. Responses to the RFI will not serve as proposals, bids, or offers, which could be accepted by the Government to form a binding contract. The Carl T. Hayden VA Medical Center, 650 E. Indian School Road, Phoenix, Arizona is conducting market research to determine the capabilities of providers to fulfill our Ophthalmology Service requirements to eligible veterans under contract. The components of services include: (1) Eye Clinic Services to be performed at the Carl T. Hayden VA Medical Center, and (2) Surgical Services to be performed at the contractor_s facility, to be located in close proximity to the VA Medical Center. The estimated annual requirements are as follows: 1. Eye Clinic _ Contractor, in collaboration with VA, will establish clinic profiles that will ensure timely initial and follow up appointments, based on 19,000 outpatient visits annually. 2. Surgical Services _ Contractor to provide highly qualified, board certified ophthalmologists who can provide a full spectrum of ophthalmology services including corneal, medical and surgical retina, glaucoma, neuro-ophthalmology, oculoplastics and general ophthalmology. Estimated Annual Surgical Cases - 620 consisting of: Cataracts 443 Retinas 69 Corneas 9 Plastics 99 Interested parties should provide information that clearly indicates your capability to meet the above requirements. If necessary, in order to maximize competition, we are open to alternative proposals to meet our requirements (i.e. separate contracts for the Eye Clinic Services and the Surgical Services).
