Description
The VA National Acquisition Center will issue RFP-797-NC-05-0024 for the Department of Veterans Affair’s VA requirements for 10-Part Laboratory Specimen Labels. The Department of Veterans Affairs intends this procurement as a sole source service disabled veteran owned small business set-aside. This procurement has an estimated annual value of $413,588.15 for the following 10-Part Laboratory Specimen Labels. Product description for the 10-Part Laboratory Specimen Labels is as follows. 1. Label size must be 4” wide by 2-3/8” high. 2. Labels must be die-cut into ten (10) parts within the above dimensions. 3. Each label must consist of three (3) rows and each of the three (3) rows must have the following specifications: a. Top row must consist of two (2) parts across that are each 2” wide by 1” high. b. Middle row must consist of four (4) parts across that are each 1” wide by ˝” high. c. Bottom row must consist of four (4) parts that are each 1” wide by 7/8” high. 4. Backing must not exceed 4-1/8” wide by 2-1/2” high. 5. Backing must be vertically perforated between labels. 6. Labels must be wound with coated side out. 7. Labels must be self-adhesive paper. 8. Labels must be laser quality. 9. Labels must be direct thermal. 10. Labels must be flexible and have the ability to adhere to curved surfaces. 11. User must be able to write on labels with a pencil, pen or marker. 12. Labels should use a “glove-proof” adhesive to avoid sticking to user’s gloves. 13. Labels must be water-proof and solvent resistant. 14. Label surface must be smudge resistant. 15. Labels must be able to be used on glass, plastic, rubber or metal. 16. Labels must be available in various ordering quantities (e.g., RO, BX, CS, etc.) to accommodate a range from small to very large labs. 17. In addition to commercial quantity per roll, rolls of at least 1,400 labels must be available. 18. Core sizes of 1” and 3” must both be available. 19. Label must adhere to surface at temperatures ranging from -80 degrees Celsius to + 50 degrees Celsius. 20. Labels must accommodate ISBT 128 bar-coding/labeling. 21. Labels must adhere to surface during centrifugation. 22. Labels must be compatible with printers used in the VA. The estimated quantities listed above are for VA only. Offerors must be able to provide the 10-Part Laboratory Specimen Labels as listed. The contract period will be for one year plus four one-year option periods. The contract items will be distributed through delivery orders to VA facilities. If offeror is not a manufacturer, a signed agreement between the offeror and the manufacturer must be submitted with offer and remain in effect for the duration of the contract period including option years. This solicitation may be considered as a bundled procurement and the public is invited to submit comments to the Contracting Officer listed below. The solicitation will be available for downloading, on or about the issue date of April 7, 2005 from the VA Office of Acquisition and Material Management’s OA&MM internet web site at http://www.va.gov/oamm/busopp/sols.htm.
