- SIMPLIFIED ACQUISITION PROCEDURES DATE: JUNE 16, 2005 REQUISITION NO: 05M000109 INTENDED SOURCE: JOHNSON INSTITUTE PERIOD OF PERFORMANCE: DATE OF AWARD THROUGH ONE YEAR PURPOSE: THE DEPARTMENT OF HEALTH AND HUMAN SERVICES/ SUBSTANCE ABUSE AND MENTAL HEALTH SERVICES ADMINISTRATION/CENTER FOR SUBSTANCE ABUSE TREATMENT THROUGH THE PROGRAM SUPPORT CENTER, INTENDS TO AWARD A SOLE SOURCE SIMPLIFIED ACQUISITION TO JOHNSON INSTITUTE. CLOSING DATE IS JUNE 30, 2005. THE SPECIFIC OBJECTIVE OF THIS PROCUREMENT IS: BACKGROUND THE DEPARTMENT IS REQUIRED TO PROVIDE THE NATIONS FAITH-BASED SERVICE PROVIDERS WITH ALL THE COLLABORATIVE RESOURCES AND SUPPORT NECESSARY TO IMPROVE AND EXPAND TREATMENT AS MANDATED BY H.R. 4923. THIS CONGRESSIONAL MANDATE AND PRESIDENT EXECUTIVE ORDER IN JANUARY 2001, AND THE PRESIDENT'S FAITH INITIATIVE ACCESS TO TREATMENT RECOVERY GRANTS ISSUED IN JUNE 2004, AUTHORIZED BY SECTIONS 501(D) (5) AND 509 OF THE PUBLIC HEALTH SERVICE ACT, AS AMENDED AND THE SECRETARY OF D.H.H.S, MIKE LEAVITT, 500 DAY PLAN ISSUED MAY 2005, CREATES A COMPELLING NEED TO PRODUCE THIS PROCUREMENT IN THE TIME FRAME INDICATED. ALCOHOL AND OTHER DRUG ABUSE AND MENTAL HEALTH PROBLEMS ARE AMONG AND SOCIETY'S MOST PERVASIVE MEDICAL AND SOCIAL CONCERNS. INDIVIDUALS, FAMILIES, AND LOCAL COMMUNITIES CONTINUE TO BE SERIOUSLY AFFECTED BY SUBSTANCE ABUSE AND MENTAL HEALTH CHALLENGES. HISTORICALLY, GRASSROOTS FAITH AND COMMUNITY ORGANIZATIONS HAVE RESPONDED TO THESE ISSUES OPERATING ON SHOESTRING BUDGETS AND WITHOUT FEDERAL, LOCAL AND STATE SUPPORT. SINCE 1992, SAMHSA HAS ACTIVELY ENGAGED AND SUPPORTED FAITH-BASED AND COMMUNITY ORGANIZATIONS INVOLVED IN SUBSTANCE ABUSE AND MENTAL HEALTH SERVICES. IN JANUARY 2001, PRESIDENT GEORGE W. BUSH SIGNED AN EXECUTIVE ORDER, WHICH EXPANDED THE 1996 CHARITABLE CHOICE LEGISLATION ENACTED UNDER PRESIDENT CLINTON'S ADMINISTRATION. THE EXECUTIVE ORDER REQUIRES HHS AND OTHER FEDERAL AGENCIES TO IDENTIFY AND ELIMINATE REGULATORY AND PROGRAMMATIC BARRIERS THAT WOULD PREVENT FAITH BASED AND GRASSROOTS COMMUNITY ORGANIZATIONS FROM APPLYING FOR AND SUCCESSFULLY RECEIVING FEDERAL GRANTS OR CONTRACTS, AND THE EFFECTIVE DELIVERY OF SERVICES IN LOCAL COMMUNITIES. IN 2000 THE SAMHSA HAD UNDERTAKEN A TECHNICAL ASSISTANCE INITIATIVE TO ASSIST GRASSROOTS COMMUNITY AND FAITH BASED ORGANIZATIONS WITH THE FUNDAMENTALS OF WRITING SUCCESSFUL FEDERAL GRANTS AND ENHANCING TREATMENT AND PREVENTION SERVICE DELIVERY. FROM 2001-2004, SAMHSA HAS PROVIDED TECHNICAL ASSISTANCE TO OVER 6500 INDIVIDUALS REPRESENTING FAITH AND COMMUNITY ORGANIZATIONS AND HAS COLLABORATED WITH COMMUNITY INTERMEDIARY ORGANIZATIONS TO FACILITATE THESE ACTIVITIES. FAITH AND COMMUNITY BASED ORGANIZATIONS HAVE BENEFITED FROM THIS TRAINING AND AS A RESULT HAVE BEEN RECIPIENTS OF VARIOUS FEDERAL, STATE AND LOCAL GRANTS. WHILE THIS IS THE CASE, MANY GRASSROOTS ORGANIZATIONS HAVE HAD DIFFICULTY SUCCESSFULLY APPLYING FOR FEDERAL GRANT FUNDS BECAUSE OF SUCH REASONS AS WEAK INFRASTRUCTURES, INADEQUATE CAPACITY TO DELIVER SERVICES, AND THE LACK OF COLLABORATIVE SUPPORTS AND PARTNERSHIPS. BECAUSE OF THIS, SAMHSA'S TECHNICAL ASSISTANCE FY 2005 WILL FOCUS ON ORGANIZATIONS THAT DEMONSTRATE OPERATIONAL READINESS TO DELIVER QUALITY TREATMENT AND RECOVERY TO COMPETE SUCCESSFULLY FOR SAMHSA GRANT AWARDS. OVER THE PAST THREE YEARS, JOHNSON INSTITUTE HAS BECOME ONE OF THE NATION'S FINEST TECHNICAL ASSISTANCE PROVIDERS AND HAS DEMONSTRATED ITS CAPACITY TO PROVIDE TECHNICAL ASSISTANCE TO FAITH AND COMMUNITY BASED ORGANIZATION THROUGHOUT THE COUNTRY TRAINING IN EXCESS OF 3500 FAITH AND COMMUNITY BASED INDIVIDUALS. JOHNSON INSTITUTE IS A NATIONAL NETWORK OF FAITH BASED ORGANIZATIONS AND NETWORKS COMMITTED TO MEETING THE NEEDS OF THE POOR AND INCREASING THE CAPACITY AMONG FAITH AND COMMUNITY BASED ORGANIZATIONS SERVING THOSE IN NEED. JOHNSON INSTITUTE IS COMPRISED OF SEASONED PROFESSIONAL WITH SKILLS IN GRANT WRITING, GRANT MANAGEMENT, VOLUNTEER MOBILIZATION, NON PROFIT ORGANIZATIONAL DEVELOPMENT, COALITION DEVELOPMENT AND SUSTAINABILITY. PURPOSE THE GOAL OF TECHNICAL ASSISTANCE ACTIVITY FOR FISCAL YEAR 2005 IS ASSIST FAITH AND COMMUNITY ORGANIZATIONS WITH SUCCESSFULLY COMPETING FOR SAMHSA GRANTS AWARDS. STATEMENT OF WORK GENERAL REQUIREMENTS 1. THE CONTRACTOR SHALL PERFORM THE TASKS SET FORTH IN THIS DOCUMENT. SPECIFIC REQUIREMENTS TASK I: MEETING WITH GOVERNMENT PROJECT OFFICER TASK 1:1 THE CONTRACTOR SHALL WITHIN FIVE DAYS OF THE EFFECTIVE DATE OF THE AWARD, MEET WITH THE GPO TO REFINE TASKS AND ENSURE UNDERSTANDING OF ALL ACTIVITIES. AT THIS MEETING OR BEFORE, THE CONTRACTOR SHALL PROVIDE TWO COPIES OF A DRAFT WORK PLAN AND (TA) EVALUATION PLAN. TASK 1:2 THE CONTRACTOR SHALL, WITHIN 10 WORKING DAYS FOLLOWING THE INITIAL MEETING, PROVIDE COPIES OF A FINAL WORK PLAN, FINAL TIME LINES AND SCHEDULES, BUDGET PROPOSAL AND MATERIALS/ MANUALS TO BE PROVIDED. IN ADDITION TO THE FIRST MEETING THE CONTRACTOR SHALL HAVE WEEKLY OR BI-WEEKLY TELEPHONE CALLS TO DISCUSS PROGRESS TO DATE, ISSUES REQUIRING CLARIFICATION, PROBLEMS AND NEXT STEPS. TASK 1:2 THE CONTRACTOR SHALL TRAVEL TO WASHINGTON D.C. TO MEET WITH GPO OR OTHER DESIGNATED INDIVIDUALS TWICE DURING THE YEAR. TASK 2 THE CONTRACTOR SHALL CONDUCT THREE-FOUR (1 ? TO 2 DAYS) TECHNICAL ASSISTANCE EVENTS (WORKSHOPS AND SEMINARS) TO BUILD CAPACITY AS WELL AS PREPARE TEN OR MORE FAITH AND COMMUNITY BASED ORGANIZATIONS TO COMPETE FOR SAMHSA GRANT FUNDS IN THE STATES OF IDAHO AND WYOMING. IN ADDITION, PROVIDE TECHNICAL ASSISTANCE TO SAMHSA IDENTIFIED FAITH AND COMMUNITY GROUPS UPON REQUEST BY THE GPO. WORKSHOPS/ TRAINING EVENTS MAY INCLUDE TOPICS SUCH AS; CAPACITY BUILDING, LEADERSHIP AND INFRASTRUCTURE DEVELOPMENT, FISCAL MANAGEMENT AND AUDITING, PROGRAM EVALUATION, GRANT WRITING, STRATEGIC PLANNING, LICENSURE AND CERTIFICATION AND HIV/AIDS. TASK 2:1 THE CONTRACTOR SHALL SECURE MEETING SITES AND HOTEL ACCOMMODATIONS, PROVIDE PRE-MEETING LOGISTICAL SERVICES, PREPARING, DEVELOPING AND OBTAINING MEETING MATERIALS, SUCH AS REGISTRATION FORMS, BROCHURES, INVITATION LETTERS, AGENDA, PARTICIPANT LISTS, PREPARING THANK YOU LETTERS, AND PROCESSING PARTICIPANT AND SPEAKER REIMBURSEMENTS. GPO SHALL APPROVE LOGISTICAL PLANS, BROCHURES, INVITATION LETTER, AGENDA, PARTICIPANT LIST, MEETING FACILITATION GUIDANCE MATERIALS AND THANK YOU LETTERS. TASK 3 THE CONTRACTOR SHALL DEVELOP A DIRECTORY OF FAITH-BASED AND COMMUNITY-BASED SUBSTANCE ABUSE AND MENTAL HEALTH SERVICE PROVIDERS FOR EACH GEOGRAPHIC AREA INCLUDING THE WHOLE RANGE OF PREVENTION, RECOVERY, AND TRANSITIONAL PROVIDERS. TASK 4 THE CONTRACTOR SHALL ADMINISTER THE SAMHSA'S GPRA PARTICIPANT SATISFACTION AND FOLLOW-UP SURVEYS TO EVALUATE TECHNICAL ASSISTANCE ACTIVITY, INPUT SURVEY DATA INTO SAMHSA'S DATA BASE AND PROVIDE A WRITTEN ANALYSIS OF SURVEY RESULTS. THE GRPA FORM HAS BEEN APPROVED BY OMB. TASK 5 THE CONTRACTOR SHALL SUBMIT A MONTHLY REPORT (ONE PAGE) CONCERNING THE PROGRESS OF THE PROJECT BY THE TENTH (10TH) OF THE FOLLOWING MONTH. THE REPORT SHALL INCLUDE MEETING SURVEY SUMMARIES; SUMMARY OF ACTION STEPS TAKEN TO EXECUTE MEETINGS INCLUDING SELECTION OF PRESENTERS, PROBLEMS ENCOUNTERED AND RESOLUTIONS, DESCRIPTION OF PARTNERSHIPS AND DISCUSSION OF OUTCOMES. TASK 6 THE CONTRACTOR SHALL SUBMIT RESUMES OF STAFF, TRAINERS/ CONSULTANTS TO THE GOVERNMENT PROJECT OFFICER (GPO) FOR APPROVAL. THE STAFF, TRAINERS AND CONSULTANTS SHALL BE CULTURALLY APPROPRIATE, DIVERSE AND REFLECT THE POPULATIONS TO BE SERVED. THE GPO SHALL RESERVE THE RIGHT TO IDENTIFY EXPERT CONSULTANTS/ TRAINERS ON SPECIFIED TOPICS WHEN DEEMED NECESSARY. TASK 7 THE CONTRACTOR SHALL SUBMIT A FINAL REPORT WRITTEN 60 DAYS AFTER THE FINAL SERIES OF TRAINING ACTIVITIES. THE REPORT SHALL INCLUDE PRE AND POST CUSTOMER SATISFACTION MEETING SURVEY SUMMARIES; SUMMARY OF ACTION STEPS TAKEN TO EXECUTE MEETINGS INCLUDING SELECTION OF PRESENTERS, PROBLEMS ENCOUNTERED AND RESOLUTIONS, DESCRIPTION OF PARTNERSHIPS WITH OTHER AGENCIES, STATE GOVERNMENT OR FOUNDATIONS, DEMOGRAPHICS OF PARTICIPANTS, DISCUSSIONS OF OUTCOMES INCLUDING DESCRIPTIVE CHART REPRESENTATIONS, NAMES OF STAFF AND CONSULTANTS USED, PARTICIPANTS LIST AND RECOMMENDATIONS FOR FUTURE MEETINGS. SOLE SOURCE DETERMINATION IS BASED UPON THE FOLLOWING: THE JOHNSON INSTITUTE IS A NATIONAL, 40-YEAR-OLD, NON-PROFIT ORGANIZATION DEDICATED TO IDENTIFYING BARRIERS TO ADDICTION RECOVERY AND REMOVING THEM. IT IS THE OLDEST AND MOST SIGNIFICANT CULTURAL SPECIFIC AND RECOVERY ORIENTED ORGANIZATION DIRECTED AND LED BY PERSONS WITH INDIVIDUAL AND FAMILY RECOVERY EXPERIENCE. SINCE THE FOUNDING BY VERNON JOHNSON, D.D., AND IRENE AND WHEELOCK WHITNEY, ITS BOARD AND STAFF HAVE BEEN PREDOMINATELY PEOPLE IN RECOVERY, WHOM THE LITERATURE INDICATES BEST RELATE AND COMMUNICATE WITH PERSONS IN RECOVERY AND FAITH-BASED PROGRAMS. THE JOHNSON INSTITUTE'S FAITH PARTNERS MODEL OF CONGREGATIONAL TEAM MINISTRIES FOCUSES ON ADDICTION PREVENTION, EDUCATION, TREATMENT REFERRAL, AND RECOVERY SUPPORT. IT IS A UNIQUE AND SPECIALIZED SERVICE PROJECT AND WAS CRAFTED AND PILOTED THROUGH A PARTNERSHIP THAT INCLUDED FUNDING BY THE JOHNSON INSTITUTE AND CENTER FOR SUBSTANCE ABUSE TREATMENT. THE JOHNSON INSTITUTE, OPERATING THROUGH ITS RUSH CENTER, IS ENGAGED IN A NATIONAL IMPLEMENTATION OF THIS UNIQUE MODEL. EACH TEAM, THOUGH AUTONOMOUS AND SELF-MANAGED, REQUIRES THE TRAINING, MATERIALS, AND TECHNICAL ASSISTANCE DEVELOPED THOUGH THE PILOT PHASE BY THE JOHNSON INSTITUTE, WITH SUPPORT OF CSAT. THIS ORGANIZATION IS UNIQUELY QUALIFIED TO PROVIDE THE REQUIRED SERVICES. JOHNSON INSTITUTE IS THE ONLY VENDOR WITH THE REQUIRED CULTURAL KNOWLEDGE, COMPETENNCE, SKILLED IN RECOVERY AND AVAILABLE AT THIS TIME TO PROVIDE THE SERVICE IN THE REQUIRED TIME FRAME. THE PROPOSED SIMPLIFIED ACQUISITION IS FOR SERVICES FOR WHICH THE GOVERNMENT INTENDS TO SOLICIT AND NEGOTIATE WITH ONLY ONE SOURCE (FAR 6.302-1). A DETERMINATION BY THE GOVERNMENT NOT TO COMPETE THIS PROPOSED SIMPLIFIED ACQUISITION BASED UPON RESPONSES TO THIS NOTICE IS SOLELY WITHIN THE DISCRETION OF THE GOVERNMENT.
