Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested from GSA HOLDERS ONLY and a written solicitation will not be issued. Solicitation W9133L-05-R-DEFIB is issued as a Request for Proposal (RFP) and will result in a firm fixed price contract. AWARD WILL BE MADE ON AN ALL OR NONE BASIS. Provisions and clauses incorporated in W9133L-05-DEFIB are those in effect through Federal Acquisition Circular (FAC) 2001-04. The solicitation includes the following Contract Line Items (CLIN). CLIN 0001 Defibrillators QUANTITY 3 EA ZOLL model 20100010102011320 Includes: LCD screen showing voice prompt message, device advisory messages, elapsed time, shock count and chest compression graph. Supplied with Public Safety pass cover, soft case, operators guide and five (5) year limited warranty; two sets of CPR-D pads: one p iece defibrillation and CPR system with compression, depth, and rate sensors. Supplied with glove barrier mask, scissors, razor, wet wipe and dry wipe. Four-year shelf life. Special long life armor pack for storage in soft pack carry case. Two sets o f type 123 Lithium batteries: quantity of ten with storage sleeve-ZOLL data review software for windows: network free download (BRAND NAME OR EQUAL) CLIN 0002 Flush Wall Mounting Box QUANTITY 3 EA ZOLL model number 8000-0811 (BRAND NAME OR EQUAL) CLIN 0003 ZOLL AED Plus training unit QUANTITY 1 EA ZOLL model number 8008-0104-01(BRAND NAME OR EQUAL) Supplied with training remote control, hand held cord, one set CPR-D training electrodes, one pair of replacement gels, Operators Manual and Administrative Guide. DELIVERY 30 DAYS AFTER RECEIPT OF ORDER EVALUATION PROCESS FOR AWARD: Award will be made to the OFFER THAT PROVIDES THE BEST VALUE AND SUCESSFUL PAST PERFORMANCE TO THE GOVERMENT. All quotes will be evaluated by Technical Capability, Past Performance and Price with Technical capability being r ated more important than Past Performance and price. EVALUATION CRITERIA: All submitted offors will be evaluated to ensure their submitted item meets the following Technical criteria: All defibrillators must have Single pad application Pad shelf life greater than 3 years Non-proprietary battery Capability to show an EKG rhythm on the LCD screen High rating on dust and particle and water ingress Capable to give audio and visual feedback on depth and rate of compressions during CPR. Must show (3) three successful deliveries ie (1) on time delivery, (2) satisfied customers, (3) at least (4) four completed orders in the last (3) three years. Price: submit a firm fixed price quote for the above listed CLINS. Offeror shall include with their proposal a completed copy of the provisions for the following FAR and DFAR clauses: FAR 52.212-1, Instructions to Offerors Commercial, FAR 52.212-3 Offeror Representations and Certifications Commercial Items along with the firm DUNS number.
