- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-04. This solicitation has been set-aside 100% for small b usiness. The NAICS code is 541890. This is a detailed description of the Camo Bracelets that are needed: Silicone Camo Bracelets, 100% synthetic silicone rubber, green camo bracelets 202mm x 12mm (wide) x 1.95mm (thick) with Defend Freedom debosses logo on the outside and the National Guard web site address embossed on the inside (www.1-800-GO-GUARD.COM). Submit samples with proposals by mail to LANG-J8-P, ATTN: Brenda Oser, Bldg 39, Jackson Barracks, New Orleans, LA 70146-0330 not later than 2:00 p m CDT on June 27, 2005. ALL SAMPLES AND PROPOSALS MUST BE MAILED. NO FACSIMILES WILL BE ACCEPTED. The following FAR clauses are applicable to this combined synopsis/solicitation: 52.212-1, Instructions to Offerors-Commercial Items; 52.212-2, Evaluatio n-Commercial Items; 52.212-3, Offeror Representations and Certifications-Commercial Items; 52.212-4, Contract Terms and Conditions-Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items ; 52.247-34, FOB Destination. The following information is mandatory and must be provided in order for proposal to be deemed responsible: Solicitation Number W912NR-05-R-0001; Vendor name and address; Vendor phone and fax number; Vendor point of conta ct; Vendor Duns and Bradstreet Number and Cage Code.
