Description

The contractor shall provide all facilities, supplies, equipment and pharmaceuticals, including Scheduled drugs I, II, III, IV and V in accordance with Code of Federal Regulations (CFR) 40 (Protection of Environment) and CFR 49 (Department of Transpo rtation), Drug Enforcement Agency (DEA), Occupational Safety and Health Administration (OSHA), Department of Defense (DoD), Department of Veterans Affairs (DVA) and Food and Drug Administration (FDA). The Contractor shall be responsible for obtaining and maintaining throughout the period of performance all required permits, certificates and licenses required by federal, state and local authorities. This includes any permit and licenses required to be a hazardous waste generator. This is a one time service for the return and/or destruction and credit of non-usable pharamaceuticals. The Contractor shall comply with all federal, state and local laws governing the receiving, storage and disposition relati ng to all materials received. The contractor must schedule an on-site appointment within five (5) business days of notification of a request for service by the Contracting Office. This is a sole source procurement. Copy of RFQ is avaiable on FACNET and at https://acquisition.army.mil. All offers are due by 10:00 A.M. Hawaii Standard Time (HST) on Thursday 30 June 2005.
