Description

(i) This is a combined synopsis/solicitation for the procurement of commercial items prepared in accordance with the format in Subpart 12.6 and Part 13 Simplified Acquisition Procedures, as supplemented with additional information included in this no tice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation document and incorporated provisions, clauses and addenda are those in effect through FAC 2005-4 dtd 8 June 2005 and DFAR Supplement, current to DCN 20050606, dtd 6 June 2005. (iii) This Request for Quotes (RFQ) number is W91YOU-05-T-0035 and the place of delivery is Bassett Army Medical Center, Logistics Division, Fort Wainwright, Alaska. NOTE: Delivery location is different from Contracting Office annotated above. Quote shall include FOB Destination shipping costs to Fort Wainwright, Alaska. (iv) This solicitation is 100% set-aside for Small Businesses. (v) The applicable NAICS is 423450 with a size sta ndard of 100 employees. (vi) All prospective quoters must be actively registered in the Central Contractor Registration (CCR) at www.ccr.gov. (vii) This RFQ has one (1) CLIN. CLIN 0001 is for two (2) each, Electric Elevation Adjusting Chiropractic Table, Elite Model EA-3, Brand Name or Equal. The salient characteristics of this model are as follows: (1) Variable elevation height from 20 - 27; (2) 21-24 wide table surface; Cervical drop with forward motion; (3) Knee break (adjusts up 20 degrees, down 1 0 degrees) with ankle rest extension; (4) Electric elevation motor rated to 500 pounds; and (5) elevating pelvic and lumbar sections (each tilt upward to 15 degrees). Preferred color is Burgandy to match existing exam room color scheme. Other colors will be acceptable with prior approval. Vendor must provide color choices at time of RFQ submission (viii) Delivery Information: MUST BE delivered to FOB Destination no later than 20 July 2005. (ix) The government intends to award a purchase order to the r esponsible, responsive quoter whose quote will be the most advantageous to the Government based on price and technical acceptability of items quoted. (x) The following FAR and DFAR provisions and clauses apply to this acquisition and are available in full text at http://farsite.hill.af.mil/ or www.arnet.gov: FAR 52.212-1, Instructions to Offerors - Commercial Items; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items; FAR 52.211-6, Brand Name or Equal Specific clauses cited in FAR 52.212-5(b) are applicable to this acquisition: FAR 52.222-3, Convict Labor; 52.222-19, Child Labor  Cooperation with Authorities and Remedies; 52.225-13, Restrictions on Certain Foreign Purchases; FAR 52.232-33, Payment by Electronic Funds Transfer - Central Contract or Registration; and FAR 52.247-64, Preference for Privately- Owned US-Flag Commercial Vessels (Alt I). DFAR provisions and clauses include the following: DFAR 252.212-7000, Representations and Certifications and DFAR 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders App licable to Defense Acquisitions of Commercial Items. Specific clauses cited in DFAR 252.212-7001(b) is as follows: DFAR 252.232-7003, Electronic Submission of Payment Requests. (xi) The following addenda are incorporated: FAR 52.212-4 in supplemented by the required use of WAWF (Wide Area Work Flow) to submit electronic invoices to the DoD payment office (DFAS).
