- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation AG-67T0-S-05-0005 is issued as an Request For Quotes. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-03. Numbered notes: 1 – NAICS is 621999, Size standard is $8.5 Million. SCOPE OF CONTRACT: This contract requires approximately 150 health fair blood screenings, 50 prostate specific antigen testing, and related work in compliance with its terms, specifications, and provisions. This includes furnishing labor, equipment, supervision, transportation, operating supplies, and incidentals. LOCATION AND DESCRIPTION: Blood screenings to be done in Custer, SD and Spearfish SD - Dates to be negotiated. TECHNICAL SPECIFICATIONS: Blood screenings to include the eight tests following: ChemScreen/TSH/LDL/Ferritin Reflex (32 panel screening includes: TSH, Glucose,Iron, Liver, Kidney, Bone, Muscle & Cholesterol (Direct LDL) with Ferritin Reflex when abnormal iron results occur.); CBC (Complete Blood Count - This screening presents a general picture of a person’s overall blood health and checks for infection, anemia and early signs of leukemia); PSA (Prostate Specific Antigen - It is recommended that men over fifty have this screening performed annually. Men should begin getting a baseline in their forties and even earlier with a family history of Prostate Cancer/ Disease); Hemoglobin A1c (Provides a record of your average blood glucose level over the past 3 months and is non fasting screening. The glucose screening which is included in the ChemScreen is a fasting test and provides a moment in time picture of your glucose level. Together they provide a much clearer view and will help your physician in assessing your health, health risks and concerns. This screening is recommended for anyone with a history of diabetes in their family.); Homocysteine An amino acid produced by the body. Elevated levels are correlated with increased risk of heart disease. Screening is recommended for those with a family history of heart or vascular disease, smokers, and overweight individuals. Homocysteine is always present and always elevated in heart attacks. There are different ways to reduce and control a person’s homocysteine level.); Cardio C-RP Elevated levels of C-reactive protein can indicate artery inflammation. Inflammation may cause cholesterol deposits to break off and clog an artery. This screening is NOT recommended for those who have an inflammatory disease or for women that are not yet post menopausal.); Colorectal Kit A take home kit that is self administered (tests for hidden blood in the stool, an early warning signal of colorectal disease(s) or cancer). Results are sent to participants personal physician; Osteoporosis Heel Scans (Achilles Express Ultrasonometer heel scan. Evaluates your risk of bone fracture.) Test results are to be sent directly to the employee’s home address – confidentiality is of extreme importance. EVALUATION: 52.212-1 applies: Provide copy of certification for blood testing laboratory, references from companies you have provided service within the last three years, and pricing of tests. Provide a completed copy of 52.212-3 Offeror Representations and Certifications with your offer. 52.212-4 Contract Terms and Conditions – commercial Items applies. 52.212-5 Contract Terms and Conditions…. Yes, that is part of the contract. Q. What is the turn around time for the blood tests? A. What is needed to complete the tests and submit the report – respond in you quote. Q. Where are the clauses and forms that are referenced? A. You can find them at www.arnet/gov/far/ Q. When will these be held and do they need to be held at both locations on the same date? A. We will negotiate this with the contractor upon award. We are looking to accomplish this within the next month. The locations do not need to be on the same date. Q. Is the ultrasound test required? A. Only if it is required to complete the blood drawn test, if it is a recommendation for additional tests, this should be on the report. Q. Can this be faxed? A. Yes, fax number (605) 673-9350.
