Description

The VA Healthcare Network Upstate New York (Albany, Bath, Canandaigua, Rochester Out Patient Clinic, Syracuse, and Western New York) Medical Centers are soliciting bids to provide ear mold services to eligible veterans. Anticipated period of performance is base year, August 1, 2005 through July 31, 2006 plus four (4) one year options at the discretion of the Government in accordance with all specifications, work statement, terms and conditions contained herein. All qualified offerors/firms who believe they can satisfy the solicitation are invited and encouraged to provide their proposal. Offers shall be evaluated for fair and reasonable pricing, technical capability to perform services and satisfactory past performance record. The Government anticipates award of multiple commercial item contracts from this solicitation to the responsible offerors whose offers conform to the solicitation and will be most advantageous to the Government, price and other factors considered. This is a Firm Fixed Price contract and at least 50% of the contracts awarded will be set-aside for small business. The preferred method of payment will be Government Purchase/Visa Credit Card and the Contractors must be capable of accepting it for payment. The size standard for North American Industry Classification System (NAICS) code 339113 is 500 employees. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications--Commercial Items. Electronic version and hard copy of the solicitation will be available on or about June 23, 2005. Electronic solicitation may be obtained at the following URL: http://www/fedbizops.gov. Requests for hard copy must be in writing to the Point of Contact and may be faxed to (716) 862-8893. No telephone requests will be accepted.
