Description
This is a combined synopsis/solicitation for commercial items/services prepared in accordance with the format in FAR Subpart 12.6, the test program in FAR Subpart 13.5 as supplemented with additional information included in this notice. Project identification number is BDR05TSA10T5. All vendors wishing to propose for this requirement must email the contracting officer, Tommie Rushing at tommie.rushing@gsa.gov or phone at 405-609-8084 to request appendix A and attachment A which further defines this requirement. Any questions regarding this requirement shall be submitted to the contracting officer, Tommie Rushing via email at Tommie.rushing@gsa.gov no later than 4:00 PM, 23 June, 2005. Answers to any and all questions will be posted for availability for all interested parties. Solicitation Number 7TF-05-0005 Request for Proposal. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-4. This is a full and open procurement under NAICS Code 517110 Wired Telecommunications Carriers. Contractor?s proposal to be submitted no later than 4:00 Central time, 6 July 2005. The proposal shall be emailed to the Project Manager, Sandra.brent@gsa.gov, and GSA Contracting Officer Tommie.rushing@gsa.gov. Requirement: Performance Based Statement of Work Project BDR05TSA10T5 General Services Administration For the Transportation Security Administration 1.0 Objective: The Transportation Security Administration has a requirement to install 18 analog lines at the Little Rock National Airport at 1 Airport Road, Little Rock AR 72202. Install 9 voice lines (6 of the voice lines will need voice mail), 1 fax and 8 modem lines to demarc located on the 1st floor of the Airport, customer will provide inside wiring to desktop. Lines may include the following basic features: (see Attachment A: template of line requirements) --Call forward --Call forward no answer --Call forward busy --Call pickup --Call transfer --Class of service GD6 --3-way conferencing --Out going caller ID only (ID should read U.S. Government and will not be blocked) -- Lines will have access to local, toll free, and domestic long distance (FTS2001 switch voice access). PIC/LPIC MCI. NO international dialing. -- Provide customer with 2 telephone Directories. --Provide customer with voice mail instructions 2.0 Period of Performance: From date of award - not to exceed nine months. Upon award of a new GSA local dial tone contract GSA will transition this service under the new contract vehicle. 3.0 Contract Price : Firm Fixed Price. Vendor shall provide month to month fixed pricing to include cost of features, Regulatory fees and Federal, State and local fees and taxes. 4.0 Invoicing/Procedures for Payment: GSA will provided invoicing information once award has been made. Vendor shall provide the following vendor information: DUNS numbers Tax ID number Vendor Name Mailing Address Point of Contact with contact information 5.0 Privacy Act Work on this project may require that personnel have access to Privacy Information. Personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a and applicable agency rules and regulations. 6.0 Personal Service The client has determined that use of the GSA contract to satisfy this requirement is in the best interest of the government, economic and other factors considered, and this task order is not being used to procure personal services prohibited by the Federal Acquisition Regulation Part 37.104 titled ?Personal services contract?. 7.0 Section 508 All electronic and information technology (EIT) procured through this statement of work and any resulting contract, task order, delivery order, or purchase order must meet the applicable accessibility standards at 36CFR 1194, unless an agency exception to this requirement exists. 36 CFR 1194 implements Section 508 of the Rehabilitation Act of 1973, as amended, and is viewable at http://www.section508.gov/. The contractor shall indicate for each line item in the schedule whether each product or service is compliant or noncompliant with the accessibility standards at 36 CFR 1194. Further, the proposal must indicate where full details or compliance can be found (e.g., contractor?s website or other exact location). Applicable standards are 1194.21-1194.26. 8.0 Security There are no required security clearances for this effort. 9. 0 Quality Control. The contractor shall provide a description of their Quality Control, Surveillance and Assurance plan as a part of proposal. Records/Data. All documentation, programs, data, products, and associated items used during the execution of this task order are to remain the sole property of the government. This information will remain in-house at the primary site and will be returned to the Client Representative or designated alternate at the completion of the task order. Recurring services will be provided as a result of the monthly maintenance charges. Non-recurring services will be performed on a case by case basis upon approval by the contracting officer. Recurring Services: Maintenance -Routine-Service restored within 24 hours -Routine Service Orders- Shall be accepted during contractor?s normal working hours -Priority- Service restored within 4 hours -Priority Service Orders ? Shall be worked ahead of pending routine service orders - Inventory Reports -Traffic Studies -Trunking (local and long distance) -Escalation Hierarchy Non-Recurring Services -Installation of phone lines -Disconnection of phone lines 12.0 Inspection and Acceptance The performance of the contractor will be based on their ability to deliver and support the services detailed on the following criteria: Timeliness Quality Additional explanation of the criteria and requirements associated with the evaluating the performance of the contractor is detailed in Appendix A. Performance Matrix. Proposal shall consist of the following: - Brief Technical Approach to satisfy the requirement - Past Performance references for three similar types of work. Include customer POC and phone number and a brief description of the work. - Cost proposal to be exhibited as the contract distinctive identification number, nomenclature, unit of issue, quantities, unit prices, extended prices and total cost. Vendor shall provide costs to add/delete additional lines under contract. Place of Performance ? EVALUATION?COMMERCIAL ITEMS (JAN 1999) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical/Management Approach Past Performance/Experience Price Technical and past performance, when combined, are significantly more important when compared to price. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party.
