DESCRIPTION OF SERVICES. 1.1 Contractor shall furnish all non-coin operated, commercial washers and dryers on a rental basis necessary to perform all work requirements in accordance with the terms and conditions of this contract. Contractor s hall furnish all plant, labor, material, transportation, and equipment necessary to install, maintain, repair, and relocate contractor-owned washers and dryers. Contractor shall clean vent ducts and exhaust fans for all laundry rooms, replace gas supply l ines, and hot and cold washer hoses. 1.2. The Contractor shall furnish and maintain sufficient, general service-type equipment and tools, and all parts necessary to support such equipment and tools during the term of the contract. Such equipment and parts shall be subject to inspection and a pproval of the Contracting Officer or his authorized representative (COR). 1.2. MAINTAIN WASHERS. 1.2.1. Contractor shall inspect, clean, repair, or replace water hoses and strainers. Washing machines shall have not less than an eighteen (18) pound dry weight drum capacity with three cycle (heavy, regular, and delicate) washing capabilities. Washers shall be equipped with a lint-filter or automatic lint flush system and will automatically shut off when the lid is opened or by manual shut down features. Each washer shall be able to operate on a 3-phase, 110 volt, 60-cycle, 20-Amp electrical circuit wi thout overloading under normal operating conditions. Each machine shall have hot, warm, and cold water temperature selectors with fabric softener and bleach dispensers. Machines with coin slots must be fixed to allow free-operation of machine. Washers s hall be stabilized and leveled in such a manner as to prevent noise and vibration. Washer machines shall not be grounded to water supply lines or drain line pipes but properly grounded to prevent electrical shock. 1.2.2. Contractor shall ensure that inoperable machines are repaired, replaced or changed out within forty-eight (48) hours after notification of the deficiency. Contractor shall notify the building manager and the COR when repairs will be accomplished. 1.2.3. Contractor shall place an OUT OF ORDER sign on broken machines with an expected date of repair for unserviceable equipment. 1.2.4. Contractor shall provide to the Contracting Officer and the COR a list of buildings where washers will be installed, the number installed, and a numeric accountability of each appliance with model and serial numbers in each location. 1.2.5. Contractor shall be given a thirty calendar-day phase-in period to install, connect and make fully operational all machines listed in TE  3. The contractor will submit a time line written schedule to the COR what days the appliances will be inst alled in each facility. When machines are added, removed, or relocated, the model, serial, and machine numbers of each machine will be furnished to the Contracting Officer and COR for updating existing washer and dryer list. 1.2.6. Upon request from the Contracting Officer or COR, the contractor shall remove washers within the same building, from one building to another, or from any areas where contract-furnished appliances are located for the convenience of the government an d shall replace such appliances when instructed to do so by the Contracting Officeror contracting office representative. 1.2.7. Utilities such as electricity, hot and cold water supply, and shut off valves for each appliance will be provided and maintained by the Government. Utility outlets such as hose bib connections and electrical receptacles are accessible by installi ng ample length hoses water supply attached to each machine. Contractor shall not be permitted to modify existing utility services. Contractor shall furnish electrical connections from appliances to electrical outlets, water hoses to hose bib connections, adapters, fittings, and other accessories necessary to connect machines to existing utilities. 1.2.8. Contractor shall provide names, phone numbers and or pager numbers of each contact person responsible for servicing machines. Repairs on machines during normal working hours will be accomplished between 0745 to 1645 hou rs Monday through Friday. Trouble calls received after normal working hours will be repaired the next workday. Trouble calls received on weekends and holidays shall be repaired by the next immediate workday. Contractor shall notify the building manager on status prior to departing. 1.3. MAINTAIN DRYERS AND VENT SYSTEMS. 1.3.1. Contractor shall inspect, clean, repair, or replace lint screens, gas supply lines, and compression fitting. Contractor shall provide clothes dyers with not less than an eighteen (18 cubic foot capacity) pound dry weight drum capacity. Dryers sha ll be equipped with a lint-filter and shall be inspected to ensure continuous operation. Flexible gas supply lines, adapters, connectors, couplings, and fittings shall be furnished and replaced by contractor as necessary. 1.3.1.1. Each dryer shall operate on a 3-phase, 110 volt AC, 60 HZ, 20 Amp circuit. Gas and electric dryers shall have not less than a 5.5 cubic feet capacity. Dryers shall be equipped with temperature settings for fabrics such as permanent press, norma l, delicate, and air only. Machines shall have timer capabilities for the desired fabric. Contractor shall ensure lint screens are installed in each dryer. Dryers shall be stabilized and leveled to prevent noise and vibration and be properly grounded to prevent electrical shock. 1.3.1.2. Contractor shall ensure that inoperable machines are repaired, replaced or changed out within forty-eight (48) hours after notification of the deficiency. Contractor shall notify the building manager and the COR when repairs will be accomplished . 1.3.1.3. Contractor shall provide names, phone numbers and or pager numbers of each contact person responsible for services machines. Repairs on machines during normal working hours will be accomplished between 0745 to 1645 hours Monday through Friday. Trouble calls received after normal working hours will be responded to the next workday. Trouble calls received on weekends and holidays shall be responded to the next immediate workday. Contractor shall noti fy the building manager on status prior to departing. Contractor shall provide to the Contracting Officer or COR a name and phone number of a contract person who can be notified after normal duty hours in the event machines breakdown occurs. 1.3.1.9. Contractor shall clean dust and lint accumulation from dryer vent ducts, hose connections, exhaust fans, wall vent ducts, main vent ducts, and flexible vent duct hoses quarterly. Contractor shall provide an annual schedule of duct cleaning to th e COR and Contracting Officer. Loose debris and trash behind dryers shall be cleaned up and removed. 2. PERFORMANCE REQUIREMENT SUMMARY. The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service requ ired for each requirement. These thresholds are critical to mission success as specified in Attachment: 2 Performance Requirement Summary. 2.1 ADDITIONS, DELETIONS, SUSPENSIONS, AND REDUCTIONS OF SERVICES. 2.2. The Government reserves the right to add or delete buildings in whole or in part for the services and reserves the right to increase or decrease washers and dryers in whole or in part for the services to be provided under this contract. Request to i ncrease/decreaseappliances shall be adjusted through contract modifications. Removal or relocation of washers and dryers shall be performed at no additional cost to the government. 2.3. The Government may increase the quantities indicated in section C.1.2.8 by an estimated thirty-three (33) percent due to a possible increase in the student population. 3. GENERAL. 3.1. CONTRACTOR QUALITY CONTROL (CQP) PROGRAM: 3.1.1. Contractor shall establish and maintain an effective quality control plan to ensure the requirements of this contract are provided as specified. 31.2. Contractor shall establish a quality control plan to enable the Contractor to perform sufficient inspection of all items of work, including that of subcontractors, to ensure conformance to applicable specifications with respect to the materials, wor kmanship, functional performance, and identification of all deficiencies. 3.1.3. Contractor has full responsibility for compliance with all provisions of the contract. The Government will inspect for compliance with all contract requirements in accordance with the Inspection/Acceptance, FAR 52.212-4(a). 3.1.4. Ten day after contract award and prior to performance of work, the contractor shall meet with both the Contracting Officer and other authorized Government Representatives if necessary to discuss quality control requirements. The meeting shall deve lop mutual understanding relative to details of the plan quality control operations, inspections, and the interrelationship of Contractor and Government Inspectors. 3.1.5. Contractor shall furnish the Contracting Officer, within ten (10) calendar days after receipt of notification of contract award, a quality control plan that includes procedures, and instructions. 3.1.6. Contractor shall periodically review the quality control plan to ensure the plan minimizes appliance failures. 3.1.7. Contractor shall notify the Contracting Officer, in writing, of any proposed change to the Contractors Quality Control Plan. 4. SAFETY PROGRAM: 4.1. Contractor shall be responsible for instructing his employees in appropriate safety measures and shall not permit them to place machines, or any other equipment in traffic lanes or other locations in such a manner as to create safety hazards. Approp riate signs shall be provided and placed by the Contractor to mark areas, which are slippery or unsafe. 4.1.1. Contractor shall ensure safety requirements and procedures to include employees responsibility for overall accident prevention are administered and practiced by all employees. Personal Protective Equipment (PPE) shal l be provided and worn in the performance of work requirements to all employees at contractor expense. 4.1.2. Contractor shall be fully responsible and directly liable for incidental property damage, personal injuries and or death to personnel as a direct result of faulty installed equipment installed, replaced, repaired or removed by the contractor. Con tractor shall hold the government harmless from any or all claims arising from such cause. 4.1.3. Contractor shall establish, implement, and maintain a safety program for preventing accidents and preserving the life and health of the Contractors personnel involved in the performance of this contract. The Contractor shall submit two (2) copies of his proposed safety plan to the Contracting Officer within ten (10) calendar days after receipt of notification of contract award. The Contractors safety plan shall include provisions for: a. Reporting accidents and injuries to the Contracting Officer in accordance with (IAW) AR 385-40. b. OSHA Compliance. c. Inspection of Contractors work areas for potential safety hazards. d. Furnishing Contractor personnel protective equipment and safety devices. e. Providing safety training to Contractor personnel. f. Providing first aid to Contractor personnel. 4.2. OCCUPATIONAL SAFETY AND HEALTH ACT (OSHA) REQUIREMENTS: 4.2.1. Contractor shall inform the Contracting Officer, in writing, of any OSHA violations as they are identified. Compliance with OSHA shall be the responsibility of the Contractor. The Contractor shall coordinate with the Contracting Officer to resolv e OSHA violations caused by defects in facilities. 4.2.2. Non-conforming Government facilities shall not excuse the Contractor from non-performance of services. Contractor shall make every effort to employ abatement measures or other work-around to guarantee continued contract performance. The Contractor shall coordinate with the Government when requesting variances or other exceptions from OSHA agencies. 4.3. QUALITY ASSURANCE INSPECTIONS: 4.3.1. The Government will inspect for compliance of the terms and conditions of contract requirements in accordance with the Inspection/Acceptance, FAR52.212-4(a) of Services clause, FAR 52.246-4. Inspection shall include but not limited to unscheduled, random, periodic, customer complaints, and 100% inspections. Government personnel conducting inspections shall include but not limited to the Contracting Officer and appointed by Letter of Designation an authorized Government Representative. Inspection s performed by the Contracting Officer shall take precedence over any and all inspections performed by other authorized Government Representative. 4.4. DAMAGES, EQUIPMENT, AND RESPONSIBILITY FOR WORK: 4.4.1. All tasks accomplished by washer and dryer rental personnel shall be done to preclude damage or disfigurement of furniture, building structures, and grounds. The Contractor shall be responsible for all damages to persons or property that occurs as a result of his fault or negligence in connection with the performance of his work. Surfaces, fixtures, or any other furnishings damaged by the Contractors personnel shall be repaired or replaced by the Contractor to the satisfaction of the Contracting Officer at no cost to the Government. 4.4.2. The Government will not be held responsible for any item of Contractors equipment, which may be lost, damaged, or stolen.
