- . This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is NHLBI -PS-2005-106-DDC and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2001-27. The North American Industry Classification (NAICS) Code is 333314 and the Employee Size is 500 for this requirement. This is a small business set-aside requirement. This acquisition is being conducted under simplified acquisition procedures in accordance with the provisions of FAR Part 13. The National Heart, Lung, and Blood Institute intends to purchase, an Olympus DSU Confocal attachment with scanning stage to upgrade an existing conventional Olympus microscope. The confocal attachment consists of a split aperture spinning disc which allows the use of a mercury arc lamp illumination source rather a laser to achieved good confocallity. The items required for the Olympus DSU Confodal with Scanning Stage and Optics upgrade are: (1) Item #DX-1X100, 1X2-DSU; Disk Scanning Unit for IX2, (2) Item #DS-D130, DSU-D3; Disk3, (3) Item #DS-U100, DSU-CAD; TV Adapter 1X, (4) Item #PR-500-H117P21X, 4x3 XY stage with 2mm screw, non-encoded for 1X71, (5) Item #PR-500-H30XY, Proscan controller for XY, motorized stage, (6) Item #PR-500-CS152V2, Joystick control unit for XY system, (7) Item #PR-500-H224R, Slide holder recessed, (holds one 1X3 inch slide), (8) Item #PR-500-H229, Petri Dish holder (a variable diameter from 25mm to 86mm). This purchase also includes a Hamamatsu EM digital camera which will be used with an existing Olympus spinning disc/laser trap microscope. This camera is compatible with the existing system and components, and the manufacture Olympus requires it for the system to acquire the highest quality images. The items required for the camera are: (9) Hamamatsu EM 1K X 1K Digital Camera, which consists of the following; Item # OC910002, C9100-02; EM 1KX1K CCD Digital Camera, Cable, and Power Supply System. The equipment will include installation by an authorized factory representative and on-site training. The Government?s anticipated delivery date is ninety (90) calendar days after receipt of order. The delivery and acceptance are F.O.B. Destination Bethesda, Maryland. The provisions of FAR Clause 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The provisions of FAR Clause 52.212-2, Evaluation - Commercial Items is applicable to this acquisition. The evaluation procedure is based upon the technical specifications of the required products and delivery period. The award will be made to the lowest priced, technically acceptable quote meeting the Government?s requirements. The quoter must submit an itemized price list. The Government intends to evaluate offers and award a contract without discussions with Offerors. Therefore, the initial offer should contain the offeror?s best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action and waive informalities and minor irregularities in offers received. The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The addenda to the clause read as follows: The offeror must include in their quotation, the unit price, commercial price, shipping & handling costs, the delivery period after a contract award, the prompt payment discount terms, the F.O.B. Point (Destination or Origin), the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. The Offeror must also register in the Government?s Contractor Registry System. Note: http://www.ccr.gov.
