- Medical Supplies - Potential Sources Sought Military Hospitals: (Army, Navy, Air Force) Southwest Region LINENS: Standardization Point of Contact: Cindy Ingrao, Clinical Consultant, Tri-Service Regional Business Office, Southwest Region, San Diego, CA. Telephone: (619) 532-8517. Potential sources sought to participate in the regional standardization of Linens (towels, pillow cases, bed sheets, surgical sheets, blankets, etc.). Southwest Region, a military integrated delivery network (IDN), comprised of Army, Navy and Air Force medical treatment centers in California are in the process of standardizing Linens. If your company currently holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) or an Electronic Catalog (ECAT) contract with the Defense Supply Center-Philadelphia (DSCP) on Linens, your company is eligible to participate in the standardization process. If your company is not a current DAPA holder for this product line, you are still eligible to participate in the standardization process by contacting the Defense Supply Center, Philadelphia (DSCP), Mr. John Charalabidis at (215) 737-7124 or e-mail at john.charalabidis@dla.mil to apply for a DAPA. Your company can be considered once DAPA paperwork is pending at DSCP. Please respond by Email: cingrao@nmcsd.med.navy.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than July 8, 2005. For questions, please call (619) 532-8517. 1.Does your company manufacture a full line of linen (towels, pillow cases, bed sheets, surgical sheets, blankets, etc.)? 2.Are your products available through the Prime Vendor, Owens & Minor? 3.What is your DAPA Number? 4.Under what brand(s) does your company manufacture or distribute your product? 5.Are your products available in a variety of sizes for neonatal, pediatric and adults? 6.What colors are your products available in? 7.Please list the styles for each of the products your company offers? 8.Does your company have the capability to apply lettering to your products? 9.How many washes is your lettering guaranteed not to fade? 10.Please supply the following for each product line your company offers: Product Type, Name of the Line, Size, Weight of Fabric (Lightweight - Moderate Weight ? Heavyweight), Thread Count, and Fabric Blend 11.Are the fabrics you use wrinkle resistant, stain releasing, and fade resistant? 12.Are your fabrics flame retardant and do they meet fire safety requirements established for hospital linen? If so, please provide documentation your products meet these standards for patient safety. 13.Do your products have reinforced seams and thread to avoid unraveling? 14.Is there anything else you would like to mention about your products? 15.What types of educational tools or materials do you have for this product line? 16.Does your company provide customer service 24/7? 17.Is there any history of backorders and/or recalls for this product group?
