Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Far Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation: proposals are being requested and a written solicitation will not be issued. Requirement set-aside for small business concerns only. The NAICS for this combined synopsis/solicitation is 238330, size standard is $12M. RFQ number FA4801-05-T-0377 as a request for quote. This document incorporates provisions and clauses that are in effect through Federal acquisition circular FAC 23001-24, effective 19 Jul 2004. The statement of work below describes the government requirement. The government intends to award one lump sum for the following: Provide all labor, materials, tools, equipment and transportation to install government provided new carpet. The contractor shall provide cushion, Type, ? Dqut Foamex # 31-11 CX 2580, 80-oz.. This is a request for quotes on Installation of government owned Carpeting to be installed in the Officers Club on Holloman AFB NM 88330. The Officers Club Bldg 531 will be closed for business from July 11 through July 29. Asbestos removal needs to be accomplished by the government in some of the work areas and will start on July 20, 2005. This will need to take place prior to the commencement of carpet installation in those areas. The ballroom is not included in the area needing asbestos removal and the work in that area may be performed while the asbestos is removed by the government from the other areas. All carpet work should be accomplished by July 29, 2005 depending on completion of asbestos removal. All quotes must be e-mailed to joyce.frost@holloman.af.mil or faxed to (505) 572-2107 with attention to Joyce. Quotes are required to be received no later than 12:00 MST, Wednesday, 6 July 2005. All quotes shall be marked with Request for Quotation Number, Date, and Time. Facsimile quotations will be accepted. Site Visit time and Statement of Work w/Specifications follows contract clauses. FOB: Destination for delivery: Holloman AFB, NM 88330. Required Period of Performance: July 11, 2005 ? July 29, 2005. Responsibility and Inspection: supplier is responsible for the performance of all inspection requirements and Quality Control (QC). The provision at FAR 52.212-1 Instructions to Offerors-Commercial Items, applies to this acquisition. FAR 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The Government will award a firm-fixed price contract resulting from this combined synopsis/solicitation to the responsible offeror whose offer conforming to the synopsis/solicitation will be most advantageous to the Government. Award will be made based on price and past performance equally. Offerors must include a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Items, with quote. The clause at FAR 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. 52.212-4 addendum to are included. Specifically, the following clauses cited are applicable to this solicitation; DFARS 252.225-7000 Buy American Act--Balance of Payments Program Certificate; DFARS 252.225-7001 Buy American Act and Balance of Payment Program; DFARS 252.225-7002 Qualifying Country Sources as Subcontractors; DFARS 252.232-7003 Electronic Submission of Payment Requests. SITE VISIT has been scheduled for all interested parties on June 30, 2005 at 9:00 AM. All contractors will meet at the Main Gate of Holloman AFB and proceed with Contracting Officer to the site. You will need to arrive no later than 8:30 AM to clear security by 9:00 AM. Security clearance usually requires a minimum of 30 minutes to 1 hour depending on demand. A valid pictured ID is required for every person entering the base. Current Registration and proof of Insurance is required for every vehicle driven on to the base. All parties interested in attending the site visit should notify Joyce Frost at 505-572-2108 by 1200 hrs. June 29, 2005. Statement of Work Replace Carpet Bldg 531 Officers Club General requirements: Remove and replace carpet. Contractor shall provide all materials, labor and equipment required to accomplish a complete removal and installation of new cushion and carpet (double stick method) in the areas designated by the floor plans. Contractor shall field verify all measurements. Note: The carpet is government provided . 1. Committee of Fifty Room, Hallways II, Hallway III and Main Lobby areas contain ACM: The removal of the existing carpet and floor material will be conducted by the government. 2. Committee of Fifty Room, Hallway II, Hallway III and Main Lobby areas approx. 322 sq yds.: Install cushion and carpet (double stick method). As per manufacturers recommendations. ? Cushion (contractor purchased) Type; ?" Foamex # 31-11 CX 2580, 80-oz. ? Carpet (government owned) ? Type Patcraft, ? Style Intelligence, ? Color CIA 18459 00102. 3. Ballroom and Lounge (American Grill) areas. Approx 466 sq yards: Remove and replace existing cushion and carpet. Carpet and cushion type same as in Para. 2 4. Offices, ladies restroom and closets adjacent to the main lobby approx 60 sq yards: Remove and replace cushion and carpet. 5. Cove-base: All wood cove-base shall remain. Replace all carpeted fashioned cove-base with the same type of carpet as in para. 2. Install metallic edge cap molding in lieu of plastic. Submit sample for approval. 6. Contractor shall provide and install metallic state of the art transitional moldings at all transitions affected by new carpet installation. Please submit samples for approval. 7. Contractor shall not be responsible for the removal and reinstallation of furniture and or equipment. Facility managers will conduct all furniture moving. 8. Contractor shall coordinate with the facility managers all applications of work schedules and work areas where traffic boundaries apply to eliminate delays to both the facility employees and contractors alike. 9. Contractor shall dispose of all carpet and material and all associated debris daily from the Holloman AFB installation in accordance to applicable state and federal landfill laws. Working area shall be kept at a high standard of cleanliness at the end of each workday. 10. Working Hours: Normal working hours are between 07:00 AM and 4:00 PM. Monday thru Friday. Non working days are weekends and major holidays. 11. Contractor must notify the CO three days in advance before starting work. 12. Contractor's employee List: Contractor shall submit to the CO a list of all personnel and vehicles required for this project within Five days prior to start of project coordination with the Security Forces. 13. Storage: The government does not provide secured storage. The Contractor is responsible for, deliveries, proper cover and storages of all materials, tools and equipment. Materials left out in the open without proper cover from the elements are subject to be rejected and shall become the contractors responsibility. 14. Environmental Controls: The contractor shall be knowledgeable of and must comply with all applicable Federal, State, and local laws, regulations and requirements regarding environmental protection. 15. Hazard Materials: The contractor is responsible for instructing his employees of all Environmental and Hazardous Material Handling requirements. Must maintain Material Safety Data Sheets ( MSDS) Reference AFI 91-301 and OSHA requirements. (END OF STATEMENT OF WORK) HAFB 999 INSTRUCTIONS 1. This solicitation utilizes brand name descriptions. Pursuant to FAR 52.211-6, the offeror may quote on "or equal products". Offerors quoting on a "or equal " must follow the provisions of FAR 52.211-6 or risk their offer being rejected as non-responsive. 2. The Government provided measurements are ESTIMATES and are provided for informational purposes only. The Government assumes no liability related to their accuracy. Offerors shall obtain their own measurements (which can be accomplished at the site visit) and price their offers accordingly.
