Description

This is a sources sought request for information RFI for the following purpose: Conduct surveys of VA Clinical Facilities in VISN 1, New England Healthcare System to comply with NFPA 101, Life Safety Code Edition 2000 (or latest edition accepted by JCAHO at the time of the surveys). Deviations from NFPA 101 will be identified and listed by type so a plan for improvement can be developed. The following data will be presented in the four part JCAHO format for tracking status in a database: Part I Introduction, Part II Basic Building Information, Part III Life Safety Assessment, and Part IV Plan for Improvement Forms. All work shall be completed three months after award. A/E firms who meet the above requirements are invited to submit their SF 330?s (3 Copies) to demonstrate their qualifications to perform this work. Responses are due back by July 15, 2005. Firms are required to be registered and current in the Central Contracting Registration Database (CCR) http://www.ccr.gov and in Online Representation and Certification (ORCA) http://orca.bpn.gov. The SF330?s will be reviewed by a selection board to determine the top three firms. Items to be considered but not limited to, experience in this line of work, experience with the government and it?s regulations, VA Experience, JACHO experience, 3 references, to name a few. This short list of firms will then be interviewed by the selection board to determine the most qualified firm and a request for proposal, RFP will then be issued. THIS IS NOT A REQUST FOR PROPOSAL.
