- This is a combined synopsis/solicitation for commercial items or services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13 (not to exceed $100,000). This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation incorporates provisions and clauses in the Federal Acquisition Circular 2005-04. The North American Industry Classification System (NAICS) code is 62111 and the small business size standard is $8.5 million. The proposed contract is a 100% Small Business Set-Aside. Statement of Work/Specifications: The Contractor shall furnish the necessary personnel, material, equipment, services and facilities (except as otherwise specified), to perform the following statement of work/specifications: Scope of Work -General: The professional physician services required by this Request for Quotes shall be performed on-site at the Department of Commerce, David Skaggs Research Center located at 325 Broadway, Boulder, Colorado 80305. Medical services of a qualified physician requires two (2) hours per week to support the Department of Commerce, Building Management Branch ? Boulder, Employee Health Service for the period October 1, 2005 through September 30, 2006 with options to renew for four one-year terms. The period of performance shall include one basic year, October 1, 2005 through September 30, 2006, plus four one year options. Option year one: October 1, 2006 through September 30, 2007; Option year two: October 1, 2007 through September 30, 2008; Option year three: October 1, 2008 through September 30, 2009; Option year four: October 1, 2009 through September 30, 2010; Statement of Work - Specific: (a) The principle place of performance shall be at Employee Health Service located in the David Skaggs Research Building, Room GB 101, 325 Broadway, Boulder, CO 80305. (b) The Government will provide a fully equipped working area in the Health Unit where the offeror/contractor will provide professional services. (c) Medical services of a qualified physician(s) are required one day a week as follows: Tuesdays: 1:00 p.m. to 3:00 p.m. (d) The physician services to be provided are to be generally performed during the period of two (2) hours per week as outlined above. The physician(s) may, however, be required to attend occasional meetings by management if the need arises. (e) The physician shall provide medical services and perform other duties as described in this section in support of the Department of Commerce employees within the Boulder Labs. (f) The physician shall be available by telephone, at no extra cost to the Government, for advice and consultation with the nursing staff. (g) The government will provide one supervisory occupational health nurse and/or one staff occupational health nurse during the period the physician is on-site. (h) Medical supplies will be provided and paid for by the Government. (i)Vacations and other variations in the schedule will be mutually agreed upon by the Contracting Officer?s Technical Representative COTR) and the physician. (j) The Building Management Branch ? Boulder will provide administrative direction, set the objectives and provide the resources necessary for the Occupational Health Program. The physician(s), after consultation with the COTR, will be responsible for providing medical expertise on the overall medical services program. The supervisory nurse will carry out the day-to-day administrative responsibilities and execute full supervision of the staff nurses. Physician?s Duties (a) Consult with employees identified as having serious or chronic health problems as well as perform routine office consultations. b) Conduct health examinations for various purposes including initial examination following an on-the-job injury, review of status following non-work related surgery or other exams as requested by the COTR. (c) Interpret EKG results, results of laboratory procedures and other tests results as needed. (d) Conduct pre-employment examinations, medical surveillance exams and examinations as required for special assignments. (e) Evaluate private physician?s notes and orders, pre-employment exams performed by private physicians, return to work certificates and any special orders requested by the supervisory occupational health nurse or by management directive. (f) Provide guidance in health promotion programs and suggest and advise appropriate occupational medical policy. (g) Review the materials, process and procedures used by employees with special emphasis on chemical, physical, biological and radiological hazards. Based upon this review, the occupational health physician shall identify medical and environmental surveillance specifications required to track potential exposures to the staff including familiarity with employees right to know laws. (h) The physician shall demonstrate continued medical education in Occupational Medicine including participation in Regional/National Occupational Medical Societies. (i) The physician shall review and advise, as requested, on the implementation of the: A. Medical Emergency Preparedness and Disaster Plan B. Industrial Hygiene Program C. Hazardous Materials Policies and Procedures D. Americans With Disabilities Act (j) The physician shall review Worker?s Compensation cases as necessary to ensure early detection, treatment and rehabilitation of ill and injured workers and shall be familiar with compensation regulations. Physician Responsibilities (a) The physician(s) shall be responsible for providing the professional services listed above. This contract is a nonpersonal health care services contract as defined in Federal Acquisition Regulation (FAR) 37.101, under which the contractor is an independent contractor. As an independent contractor, the physician(s) shall secure all licenses, certificates and insurance necessary for the performance required under this Request for Quotes/contract. (b) The Government assumes no responsibility for acts by the physician(s) with respect to diagnosis or treatment of any employee?s health problems. Services to be performed by the physician(s) WILL NOT be subject to the supervision or control of any employee of the Department of Commerce, Building Management Branch - Boulder. With respect to how services are performed, however, the Government reserves the right to request medical opinions of other medical specialists. (c) The Contractor shall indemnify the Government for any liability producing act or omission by the Contractor, its employees and agents occurring during contract performance. (d) INSURANCE: Prior to receipt of an award, the successful offeror shall furnish to the Contracting Officer an insurance certificate showing malpractice insurance coverage. The Contractor is required to ensure that its subcontracts for provisions of health care services contain the requirements of the clause at 52.237-7, including maintenance of medical liability insurance. All insurance premiums shall be paid by the Contractor. Physician?s Qualifications (a) Qualifications, certifications and licenses of professional personnel who will perform the services required by this Request for Quotes shall meet the standards prescribed by the State of Colorado for the practice or performance within the State. Physician(s) may be subject to background investigation. (b) MINIMUM EDUCATION REQUIREMENT: Physician shall have a degree of Doctor of Medicine from a U.S. or Canadian medical school listed and approved by the Council of Medical Education and Hospitals, American Medical Association in the list published for the year of the physician?s graduation. 1. Permanent and full or unrestricted license to practice medicine and surgery in a state, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. 2. Certification in a specialty by an American specialty board approved by the Council on Medical Education and Hospitals of the American Medical Association. (c) NOTICE: The Government reserves the right to conduct a panel interview of physician(s) identified in acceptable quote(s). Such interview(s), if conducted, will be at the Department of Commerce, David Skaggs Research Center, 325 Broadway, Boulder, Colorado and will be under the administrative control of COTR Technical questions shall be submitted in writing to the Contracting Officer by fax or by e-mail. You may submit your quotation by e-mail, fax, or by mail. Quotes shall be received on or before 5:00 p.m. on August 8, 2005. Evaluation Criteria for Contract Medical Physician for Boulder Labs Health Clinic are as follows: PAST EXPERIENCE: List work experience for services that are similar in nature to the above statement of work, in order to demonstrate the Contractor?s ability to provide physician services in the health unit. WRITTEN RECOMMENDATIONS: Provide two written business recommendations from sources that the physician has worked with or been employed with for the last five years. SPECIAL LICENSING/TRAINING: In addition to medical license list any specialized training/licensing such as Occupational Health Physician, Trauma/ER Physician Specialty, etc. This is a Best Value Basis for Award of Contract: A commercial items contract will be awarded to the responsive and the responsible offeror whose quote, conforming to the solicitation, is the most advantageous to the Government for technical factors (company and personnel information), past experience and performance, and price. Technical and past performance, when combined is more important than price. Your quote should include responses to the evaluation criteria as well as pricing (fixed price of hourly rate) for the initial year of service plus the four option years as specified under the heading of Scope of Work-General. Anticipated award date will be August 22, 2005. The following provisions and clauses shall apply to this solicitation: FAR 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and there is no addenda to this provision; FAR 52.212-2, Evaluation --Commercial Items --The government will award a fixed price purchase order resulting from this solicitation to the responsible offeror whose offer will represent the best value to the government, price and other factors considered; FAR 52.212-3, Offeror Representations and Certifications --Commercial Items -- Offerors must include a completed copy of Offeror Representations and Certifications along with the offer; FAR 52.212-4, Contract Terms and Conditions--Commercial Items, applies to this acquisition and there is no addendum to this provision; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders --Commercial Items, applies to this acquisition.
