- FSC Code: 65 Title: X-Ray Unit This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. Solicitation Number V4407-0001 is issued as a Request for Quotation. This solicitation document and incorporated provisions and clauses are those in effect through FAC 05-04. The NAICS Code is 423450 and the small Business Size is 100 employees. This action is 100% Set Aside for Small Business. The Veterans Affairs Special Services (VASS) Contracting Activity has a requirement for a Multix Top/Verticle Solitaire and 90 degree tilt table x-ray unit. The quotation shall include the following part numbers: 04451022, 04483652, 04421553, 04413428, 04415084, 04490400, 03832545, 05477807, 04797036, 04490228, 04483744, 04462391, 04819756, 04451022, INS2288, RXAPPL1, RXAPPL2 and AXD_MISC_MATERIAL. Siemens Medical Solutions is the manufacturer of this equipment. Delivery, which shall occur within 45 calendar days after receipt of award, shall be made to Scott AFB, IL 62225. The provision at 52.212-1, Instruction to Offeror--Commercial Items, applies to this solicitation. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition. The Offeror shall also provide its Commercial and Government Entity code, DUNS number, and Tax Identification number. All contractual technical questions must be submitted in writing (e-mail or fax). Telephone questions will not be accepted. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov/. The Federal Contractor Program requires that any contractor receiving a contract from the Federal Government in the amount of $25,000 or more, or any subcontractor receiving a contract in the amount of $25,000 or more from such a covered contractor must file a VETS-100 Report on an annual basis.
