- FSC Code: 65 Title: Colonoscopes This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this Request for Quotation is FM5202-4266-7005. This solicitation document incorporates provisions and clauses in effect through FAC 05-04. The associated NAICS Number is 339112. The Air Force Medical Logistics Office has a requirement for the following item(s): Quantity of (2) Part Number: CF-Q180AL; Wide-angle view, innoflexTM Adjustable Stiffness Video Colonoscope with Wide Screen Image, 12.8mm diameter, 3.7mm channel, 170 degreed field of view, 168cm working length and angulation of 180 degree/180 degree (up/down) 160 degree/160 degree (right/left). To Include Biopsy Forceps FB-220U (2), MAJ-855 Auxiliary water tube (1), MAJ-215 Auxiliary water inlet cap (1). Complete with Standard Accessories. Brand Name or Equal. Olympus America is the manufacture of this equipment. Delivery, which shall occur within 45 calendar days of contract award, shall be FOB DESTINATION to Yokota Air Force Base, Japan. The Contractor is responsible for all freight charges. The provision at 52.212-1, Instruction to Offeror--Commercial Items, applies to this solicitation. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition. The Offeror shall also provide its Commercial and Government Entity code, DUNS number, and Tax Identification number. All contractual technical questions must be submitted in writing (e-mail or fax). Telephone questions will not be accepted. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov/. The Federal Contractor Program requires that any contractor receiving a contract from the Federal Government in the amount of $25,000 or more, or any subcontractor receiving a contract in the amount of $25,000 or more from such a covered contractor must file a VETS-100 Report on an annual basis.
