Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number RFQ-05-170 is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-27. The NAICS code for this solicitation is 712130 and the size standard is $6M. The Bureau of the Public Debt on behalf of the Armed Forces Retirement Home (AFRH) has an immediate need to purchase aquarium maintenance services. Requirement DESCRIPTION / SPECIFICATION / WORK STATEMENT 1.1 GENERAL REQUIREMENTS The Contractor shall provide maintenance and repair service to the two (2), two-hundred and twenty-five (225) gallon salt water aquariums in the main lobby, two (2) thirty (30) and one (1) fifty-five (55) gallon fresh water aquariums in other lobbies of the Armed Forces Retirement Home, Gulfport and one (1) three-hundred (300) gallon outdoor pond in the Long Term Care Unit courtyard. 1.1.1 Specifications A. The Contractor shall furnish all transportation, labor, supervision, tools, fish food (frozen, and dried as required and a minimum of 120 feeder fish weekly), materials (nuts, bolts, screws, plants, misc.), equipment, transportation, and management necessary to provide minimal weekly and complete twice monthly cleaning, maintenance, repair, and stocking of two (2) two-hundred and twenty-five (225) gallon salt water aquariums, two (2) thirty (30) gallon, one (1) fifty-five (55) gallon fresh water aquariums and one (1) three-hundred (300) gallon outdoor pond at the Armed Forces Retirement Home, Gulfport. The aquariums shall be maintained free of algae and salt accumulations respectively and the pond kept clean. The Contractor shall maintain the hydro-state, including water testing and necessary steps to maintain a viable habitat for stocked fish. The decor shall be cleaned or changed to maintain proper and acceptable appearance, filters and cartridges cleaned, changed and serviced as recommended by the manufacturer. This service shall be performed twice monthly within the first and third weeks. The Contractor shall also perform routine maintenance, such as cleaning algae from inside of glass, cleaning outside of glass, adding water and answering emergency service requests within12 hours. The Contractor shall perform all services on-site using trained and qualified technicians to work on salt and fresh water aquariums, ponds and related equipment. No piece of equipment will be removed from the agency compound without permission from the Contracting Officer?s Technical Representative (COTR) or the Chief of Campus Operations. The Contractor shall perform maintenance as scheduled in Section 1.1.3 and answer an unlimited number of service calls as needed to keep all equipment operational and aquatic life healthy. B. The Contractor shall schedule and arrange work to cause the least interference with the normal operations of AFRH-G. C. The Contractor shall provide service reports for the status of any replacement part on order, projected down-time and/or repair schedules, itemized list or parts used and pricing, printed name and signature of Technician performing work on any portion on this contract. D. The intent of the contract is to ensure that all equipment is being maintained and fully operational at all times. The amount of maintenance and repair, number of hours, number of service calls needed shall be determined by the needs of the equipment. A 12-hour response time is required on any service call, including those made on holidays, weekends, or after normal working hours. Overtime is not provided under the terms of this contract. If overtime is necessary, it will be the responsibility of the Contractor. If the Government is required to repair any equipment covered under this contract to prevent safety issues or further damage to the equipment, fish or a threat to personnel or property, the expense of the repair will be charged to the Contractor at prevailing rates. E. The Contractor shall perform necessary calibrations of thermometers, gauges, etc., installation, adjustments, safety and operational checks to ensure equipment meets all of the manufacturer's specifications. Preventative maintenance shall be defined as those procedures needed to keep the equipment in peak operating condition including oiling, cleaning, adjusting, calibrating, water chemistry testing and adjustment, etc. Repair will be defined as overhaul, reprocessing or replacement of constituent parts or materials that have deteriorated by action of the element or usage and have not been corrected through maintenance, or replacement of the entire unit or system if beyond economic repair. F. The Contractor shall provide new parts and components when providing maintenance and repair. All replacement units, parts, components, and materials to be used in the maintenance and repair must be compatible with the existing equipment on which is to be used, of equal or better quality than original equipment specification. G. No Government parts, services, materials, facilities, or equipment will be furnished by the Government to the Contractor except those facilities that are available to the general public (restrooms, drinking fountains, pay telephones, etc.) and those parts authorized by the COTR. No tools or other equipment shall be furnished to the Contractor. The Contractor shall maintain sufficient off-the-shelf materials and equipment on-hand to support the service call work requirements. Lack of availability of materials, equipment, or personnel shall not relieve the Contractor from the requirement of completing service call work. H. Persons authorized to place service calls against this contract include the COTR assigned to monitor this contract, the Chief, Campus Operations, and the Maintenance Supervisor. I. All areas shall be protected from damage and marring during the course of any portion of this project. This includes flooring, walls, ceilings, fixtures, moldings, lights, duct work, etc. J. The Contractor shall clean all surfaces soiled by work performed under this contract. K. The Contractor shall install any new equipment or re-install any equipment removed for repair. L. When installing new equipment, the Contractor shall arrange installation by qualified factory trained technicians regularly employed by the equipment supplier. 1.1.2 Listing of Covered Equipment A. Two (2) two-hundred and twenty-five (225) gallon salt water aquariums and associated pumps, electrical systems, lighting, stocks of fish, plants, media and rocks displayed in the tanks. B. Two (2) thirty (30) and one (1) fifty-five (55) gallon fresh water aquariums and associated pumps, electrical systems, lighting, stocks of fish, plants, media and rocks displayed in the tanks. C. One (1) three-hundred (300) gallon outdoor fresh water pond and associated pumps, electrical systems, lighting, stocks of fish, plants, media and rocks displayed in the tanks. 1.1.3 Listing of Required Maintenance A. All portions of the aquarium shall be maintained free of algae, mold, scum, salt accumulation and fingerprints. Pond shall be maintained with small amounts of algae and scum. B. All tank / pond decorations and plants, real or artificial, shall be kept clean and changed as often as needed to maintain a proper appearance. C. Testing and adjustments to water chemistry to maintain a viable habitat for stocked fish. D. Stocking of fish suitable and compatible for co-habitation. The COTR shall select from a list provided by the Contractor. The Contractor shall remove and replace dead or ill fish as required. E. Cleaning of the substrate and partial water changes. 1.1.4 Replacement of Unserviceable Equipment If the Contractor determines a piece of equipment to be beyond economical repair, he shall submit written justification to the COTR detailing the reasons that the equipment should be replaced versus repaired, giving an estimated repair cost, and giving the estimated cost of a replacement piece of equipment. The Government reserves the right to have the equipment examined by a 3rd party to determine its condition. If that examination disagrees with the Contractor?s estimate of the equipment?s condition, it shall be repaired and the cost of the examination and repair shall be borne by the Contractor. If a piece of equipment is repaired and placed back in service and subsequently breaks down again, the second malfunction shall be considered a new repair demand and not a continuation of the previous problem. 1.1.5 Qualifications of Personnel Any work performed under this contract shall be performed by personnel possessing the necessary technical qualifications, experience and training. 1.1.6 Security After a service visit is concluded, the Contractor shall lock all 12 doors to the salt-water aquariums? cabinet and return the key to the Security Desk. 1.2 GOVERNMENT-FURNISHED ITEMS ? none 1.3 CONTRACTOR-FURNISHED ITEMS The Contractor shall provide equipment, tools, supplies, materials, and services to perform the requirements of the contract as specified in 1.1.1. A. Materials and repair parts used shall be free from defects, which would adversely affect the performance or maintainability of individual components or of the overall assembly. Materials and/or parts shall be of the same quality used for the intended purpose in commercial practice. Unless otherwise specified herein, all equipment, material, and articles incorporated in the work covered by this specification are to be new and fabricated using materials produced from recovered materials to the maximum extent possible without jeopardizing the intended use. The term "recovered materials" means materials, which have been collected or recovered from solid waste and reprocessed to become a source of raw materials, as opposed to virgin raw materials. Used or rebuilt products are not allowed. B. When two or more units are required they shall be products of one manufacturer. C. Manufacturers of equipment assemblies, which include components made by others, shall assume complete responsibility for the final assembled units under warranty. 1. Components of an assembled unit need not be the products of same manufacturer, but shall be compatible. 2. Constituent parts, which are alike, shall be the products of a single manufacturer. 3. Components shall be compatible with each other, and with the total assembly for the intended service. D. Hazardous/potentially hazardous materials proposed for use during the course of this work shall be approved by the Government representative before use. The Contractor shall provide Material Safety Data Sheets for each such material. The Contractor shall not store any hazardous materials on-board the compound. Only the amount of materials required for a day?s work shall be kept on the job site. 1.4 SCHEDULING A. Performance of work shall be coordinated via the COTR or CO. Work shall be scheduled in a manner that provides minimal disruption of operations. B. All work shall be performed during AFRH-G normal working hours: Monday through Thursday, 0700 to 1630 Central, 0700 to 1530 Central on Fridays; exceptions will be requested/arranged in advance. If the Contractor desires to carry on work on Saturday, Sunday, holidays, or outside regular hours, he / she may submit a written request to the COTR for approval. 1.5 SAFETY/FIRE PREVENTION/HAZARDOUS SUBSTANCE REQUIREMENTS A. All work shall be conducted in a safe manner and shall comply with the standards of the Occupational Safety and Health Act, particularly Parts 1910 and 1926. B. The Contractor will become familiar with and comply with all the provisions of the AFRH-G Safety, Fire Prevention, and Hazardous Materials Management and Control Programs. C. The AFRH-G Safety Manager shall have the power to issue restraint or cease and desist orders to the Contractor when unsafe or harmful acts that threaten AFRH-G staff, residents, or property are observed or reported. D. The Contractor shall ensure that the safety and health of residents and staff is protected during any work performed. This includes, but is not limited to, placement of barriers, signage, isolation of fumes and vapors generated, etc. E. The Contractor shall report to the Contracting Officer, in the manner and on the forms prescribed by the AFRH-G, all accidents resulting in death, trauma, occupational disease, or damage to property. All accidents must be reported within 24 hours of the event. F. The Government will not provide safety equipment to the Contractor nor perform any testing or sampling related to the safety program. The Government shall not test, manage, administer, or otherwise become involved with the Contractor's Safety Program other than ensuring OSHA regulations are enforced. G. The Safety Manager, OSHA, state, and local agencies may conduct inspections of Contractor operations on a no notice basis and have the authority to issue cease and desist orders for violations of safety/hazardous materials standards that pose a threat to Naval Home staff, residents, or property. H. Only emergency medical care in Government facilities is available to Contractor employees who suffer on the job injury. Care will be rendered at the rates common in the surrounding community at the time of treatment. The Contractor shall reimburse the AFRH-G for treatments rendered upon receipt of a statement. I. The Contractor's equipment shall be in good repair and shall comply with all safety standards. Any equipment which the Safety Manager, or his designated representative, considers in disrepair or unsafe shall be removed from the AFRH-G compound immediately. 1.5.1 HAZARDOUS SUBSTANCES A. It shall be the responsibility of the Contractor to meet all EPA, OSHA, DOT, Federal, state, and local regulations governing the use, handling, storage, transportation, and disposal of hazardous substances and wastes. B. The Contractor shall dispose of all hazardous wastes generated by performance of any work covered under this contract. The Contractor shall not store any hazardous substances onboard the compound without the permission of the Safety Manager. C. The Contractor shall provide Material Safety Data Sheets for any chemicals or substances used in the performance of work under this contract to the Contracting Officer or his/her designated representative. The Safety Manager shall approve the use of any chemical or substance prior to its employment onboard the compound. As required by the Safety Manager, the Contractor shall provide an inventory of all chemicals or substances used in performing work under this contract. D. All containers of chemicals, solvents, or similar materials shall be clearly and conspicuously marked, preferably with a manufacturers? label that identifies the contents. Materials bearing the ICC/DOT red label shall not be used. E. No materials which are deemed by the Safety Manager to be exceptionally hazardous or which may pose an unacceptable threat to the health and safety of AFRH-G residents, staff, and/or property shall be used. 1.5.2 ENVIRONMENTAL PROTECTION A. The Contractor shall comply with all Environmental Protection Agency, Occupational Safety and Health Act, State of Mississippi, and AFRH-G laws and regulations governing environmental protection. B. Debris, rubbish, trash, garbage, non-usable materials, and hazardous wastes resulting from work under this contract, shall be disposed of by the Contractor in an off-site location on a daily basis. Expenses incurred as a result of this disposal shall be borne by the Contractor. Hazardous wastes shall be disposed of in compliance with all applicable Federal, state, and AFRH-G laws and regulations. C. Contractor work shall be conducted in a manner that protects both the indoor and outdoor air quality. D. All hazardous substance spills shall be reported to the Safety Manager and/or Security Department immediately. Contractor personnel shall be thoroughly indoctrinated in the AFRH-G Hazardous Materials Management and Control directive and shall comply with all provisions of that instruction. 1.5.3 FIRE REGULATIONS A. The Contractor shall become familiar with the provisions of the AFRH-G Fire Protection Program. B. The Contractor shall observe all requirements for handling and storage of combustible supplies, materials, wastes, and trash. C. Contractor employees shall be thoroughly indoctrinated in fire evacuation procedures and in sounding and responding to a fire alarm. The award of a contract will be made to the technically compliant, lowest price Offeror. Offerors shall submit their quotes in English by e-mail to ntprocurement@bpd.treas.gov. Quotes must be in US dollars. Quotes are due by July 18, 2005, by 2:00 p.m. EST. Only quotes meeting the requirements as specified in the statement of work will be considered by the Government. The provision at 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with their offers. The Offeror assumes the full responsibility for ensuring that offers are received at the place as delineated herein and by the date and time in item 6. All offers must be closed and sealed and must be fully identified on the sealed envelope if delivered via third party carrier or hand delivered. The Offeror assumes full responsibility for ensuring electronic quotes are formatted in accordance with BPD Security Requirements. The following file extensions are not allowable and application materials/data submitted with these extensions cannot be considered: bat, cmd, com, exe, pif, rar, scr, vbs, hta, cpl, and zip files. Microsoft Office compatible documents are acceptable. If the Offeror determines other formats are necessary, it is the Offeror?s responsibility to verify with BPD that formats are acceptable. Quote materials with unacceptable or unreadable formats may be found non responsive.
