Description
Purpose of this amendment is to extend the proposal date, change the frequency of vent ducts cleaning, and change the verbiage in para 4.4.2 of the PWS. a. Change CLIN 0004, 1004, 2004, 3004, 4004 unit of issue from monthly to quarterly; change quantity from 12 to 4. b. Proposal submission date has changed from 20 July 2005;12:00 PDT to 3 August 2005; 12:00 PDT. c. Replace page 8 of PWS para 4.4.2 with the following page 8: measures or other work-around to guarantee continued contract performance. The Contractor shall coordinate with the Government when requesting variances or other exceptions from OSHA agencies. 4.3. QUALITY ASSURANCE INSPECTIONS: 4.3.1. The Government will inspect for compliance of the terms and conditions of contract requirements in accordance with the Inspection/Acceptance, FAR52.212-4(a) of Services clause, FAR 52.246-4. Inspection shall include but not limited to unscheduled, random, periodic, customer complaints, and 100% inspections. Government personnel conducting inspections shall include but not limited to the Contracting Officer and appointed by Letter of Designation an authorized Government Representative. Inspection s performed by the Contracting Officer shall take precedence over any and all inspections performed by other authorized Government Representative. 4.4. DAMAGES, EQUIPMENT, AND RESPONSIBILITY FOR WORK: 4.4.1. All tasks accomplished by washer and dryer rental personnel shall be done to preclude damage or disfigurement of furniture, building structures, and grounds. The Contractor shall be responsible for all damages to persons or property that occurs as a result of his fault or negligence in connection with the performance of his work. Surfaces, fixtures, or any other furnishings damaged by the Contractors personnel shall be repaired or replaced by the Contractor to the satisfaction of the Contracting Officer at no cost to the Government. 4.4.2. Washers and dryers vandalized, damaged, destroyed, or stolen, by other than contractor personnel will be the primary responsibility of the government to remedy financially such property damage to the contractor. Contractor shall submit statement of charges to the tovernment for any damaged, broken, stolen orlost appliances within 10 days of observation. 4.5. CONTRACTOR MANPOWER REPORTING (CMR). The Office of the Assistant Secretary of the Army (Manpower & Reserve affairs) operates and maintains a secure Army data collection site where the contractor shall report ALL contractor manpower (including subcon tractor manpower) required for performance of this contract. The contractor shall completely fill in all the information in the format using the following web address: https:\\contractormanpower.army.pentagon.mil. The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officers Technical Representative; (2)Contract number, including task or delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e- mail address, identity of contractor employees entering data; (5) Estimated direct labor hours (including sub-contractors); (6) Estimated direct labor dollars NOTE: THIS NOTICE WAS NOT POSTED TO WWW.FEDBIZOPPS.GOV ON THE DATE INDICATED IN THE NOTICE ITSELF (15-JUL-2005); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
