Description
The Federal Bureau of Prisons is requesting offers under RFQ-51904-0001-6 for a Fitness Center. This solicitation is a Firm-Fixed Price, Commercial Items, Single Award Contract to a responsible quoter for fitness center for the United States Penitentiary (USP) and Federal Prison Camp (FPC), Pollock, Louisiana for an estimated 12-month period. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. The Government will make award to the responsible offeror whose offer conforms to this solicitation and is most advantageous to the Government. This solicitation document and incorporated provisions and clauses are those in effect thorough Federal Acquisition Circular 2005-04. All contractors doing business with the Federal Government shall be registered in the Central Contractor Registration (CCR) database. The website for registration is www.ccr.gov . The North American Industrial Classification System (NAICS) code for the service is 713940. The solicitation is unrestrictive. Faith-Based and Community-Based Organizations have the right to submit quotes equally with other organization for contracts for which they are eligible. General Information: USP Pollock has approximately 415 employees. Historical data indicates that on average 114 employees will visit the fitness center per month for roughly 660 visits per month. The membership will be in the name of the institution not the individual employees and a one time membership fee for usage will be paid within 30 days of award. The membership will cover complete usage of the fitness center (except babysitting or any additional services that are not passed on to other memberships) and will be offered to all employees regardless of usage. The effective date of the contract is expected to be October 1, 2005. The Government is requesting two different quotes. Quote #1 will be for a 12 month period and Quote #2 will be for a six month period from October to March of each year. It is anticipated the period of performance will be from October 1, 2005 through September 30, 2006. The Government reserves the right to choose the best overall proposal. Interested parties may submit additional proposals or options. Eligibility: All full-time employees will be eligible to participate in this program. Once the open enrollment has ended, the fitness center will need to provide the Executive Assistant a roster of plan participants. The fitness center will also be required to provide updates throughout the term of the contract upon our request. The institution will supply you with monthly updates of employees who have left the institution for the purpose of canceling their membership privileges. It will be the institution?s responsibility to communicate to all employees that their membership privileges will end once their employment expire. Open Enrollment: All current employees will be able to enroll from October 1, 2005 to November 1, 2005. New hires and transferring employees will be able to be added throughout the year within 60 days of their hire date (proof of hire date will be supplied by employee enrollee). The fitness facility will provide a representative to do on-site enrollments at the institution on selected days throughout the open enrollment period. In addition, the Warden will have discretion to allow employees to join that have missed the enrollment period. Family members: Immediate family members (spouse, or - year old children will be eligible and may be added to the employee?s membership for a one-time fee of $ , $ Enrollment fee, plus $ additional dues per month per member. These payments are the sole responsibility of the employee and his/her family members.
