- The Centers for Disease Control and Prevention intends to issue a sole source contract to Kaiser Foundation Hospitals, 1800 Harrison Street, 16 Floor, Oakland, CA 94612 to assess the benefits of the Polypill for preventing cardiovascular disease for Type 2 Diabetes. A recently published report of a Polypill to prevent heart attacks and stroke may be an important control measure for cardiovascular risk among persons with diabetes. The Polypill contains a statin, three blood pressure lowering drugs each at half standard dose, folic acid, and aspirin, and is estimated to reduce heart attack by 88% and stroke by 80%. The report suggests that the Polypill should be given to all persons aged 55 or over, with type 2 diabetes, or with a history of heart disease because their cardiovascular risk is higher than the general population. This research will use a mathematical and statistical simulation model based on biologic evidence and validated against clinical trials to simulate the public health implications of the Polypill. The research is part of an effort to evaluate innovative patient treatments, guidelines for quality of care, and other clinical management tools. The estimated risk reductions for the Polypill are based on meta-analysis, not on clinical trials. New biologic simulation models, such as the Archimedes model developed by Kaiser Permanente, provide simulated results that extend the results of a randomized clinical trial to a slightly different population than that considered by the trial. A biologic model can give detailed estimates of the interactions and relative risks of integrated but separate diseases, such as diabetes and coronary heart disease. A biologic model can be linked to patient treatment protocols to evaluate quality of care guidelines. This project will use Archimedes, an existing model, to answer a series of public health questions that are important to the understanding of diabetes and its clinical management. It is the belief of the Centers for Disease Control and Prevention that the Archimedes model is the only existing product that can perform our requirement. Though other bioligic models exists they they do not include diabetes and heart disease as Archimedes does. This notice is not a request for offers. All proposals received by 3:00 P.M. Eastern StandardPacific Daylight Time, August 2, 2005 will be considered by the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. A determination not to compete this proposed procurement based on response to this notice is solely within the discretion of the Government. The anticipated date of award is August 2, 2005.
