- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Texas Army National Guard requires laboratory services. The contractor shall provide all personnel, equipment, tools, materials, supervision and other items and services necessary t o perform physical exams for soldiers participating in Soldier Readiness Processing (SRP) as defined in the attached Performance Work Statement (PWS). Service locations are Grand Prairie, Austin, San Antonio, and Houston, TX. The NAICS code is 621511 wit h a standard business size of $11.5 million. The Army National Guard estimates approximately 1,300 soldiers will require this service within the next five (5) years. Contractors are to quote based on the Performance Work Statement and must complete all r equired certifications. The period of performance will be 1 October 2005 through 30 September 2006 with four (4) option years. Proposals are due by 12:00 noon CST on 6 September 2005. The following evaluation factors will be used to evaluate offers: 1) Technical Factors: Corporate Experience: Offeror must have a minimum of five (5) years experience in providing the requested service. Supervisors of laboratory services must h ave a Baccalaureate Degree in laboratory services in order to meet the Clinical Laboratories Improvement Amendment (CLIA) certification; 2) Past Performance: Offeror must have experience in performing previous contracts similar to the size of scope of th e services requested in the Statement of Work; 3) Price: A price evaluation in accordance with FAR 15.304-1 will be performed. Technical and past performance, when combined, are considered equal in determining the technical acceptability of the offerors . Of those offerors who are determined technically acceptable the offeror whom represents the overall lowest prices (base and all option years considered) will the offeror eligible for the resultant award. This solicitation will be awarded on the lowest overall price, technically acceptable offer. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). Failure to address the evaluation criteria may render offeror(s) ineligible for award. Award may be made without discussion.
