- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ). Order will be made in accordance with FAR Part 13: Simplified Acquisition Procedures. The NAICS code is 7221110: Hotels (except casino hotels) and Motels. It is the responsibility of all interested offerors to monitor the solicitation for any possible amendments and changes. Statement of Work: A. Purpose: The contractor shall provide all equipment, personnel, and services to supply nine double occupancy rooms for housing Uniformed Services University of the Health Sciences (USUHS) Medical Students during their clerkship rotations at Tripler Army Medical Center, Honolulu, HI as specified as follows: a. Contactor shall furnish nine double occupancy rooms. Rooms are to have a minimum of 200 square feet of living space (excluding bath and closet) and private bath per room. Rooms are provided with electricity, hot and cold water, individually thermostatically controlled air conditioning and trash removal. b. Facility should be within walking distance of Tripler Army Medical Center. c. Rooms must be fully furnished as specified in paragraph IV. B. Period of Performance: The period of performance will be for 12 months (base year) and four one-year option periods. The government will give the contractor a preliminary written notice of its intent to extend the contract. C. Minimum Room Specifications: Each Room must be furnished as follows: a. Double or larger bed with mattress and box springs-2 per room. b. Three-Drawer Chest or Dresser-1 per room. c. Night Stand/table with lamp-1 per room. d. Easy/Occasional chair-1 per room. e. Table with top at least 30? round-1 per room. f. Sofa/sofa bed-1 per room. g. End tables (minimum 24? x 24?)-1 per room. h. Straight back chair-2 per room. i. Additional table/floor/swag lamps-2 per room. j. Wall-toWall Carpet-All living areas except bathrooms. k. Shower curtain and liner (or equivalent enclosure)-1 per room. l. Television table/stand/or cabinet with outlet-1 per room. m. Color TV and VCR or TV/VCR combo with cable service-1 per room. n. Clothes hanging space with hanging bar extending (at least 30?)-1 per room. o. Full length mirror-1 per room. p. Blinds/shades/drapes-2 per window/patio door. q. Pillow and Pillow case-2 per bed. r. Sheets-2 per bed. s. Bedspread and Blanket-1 each per bed; extra as requested. t. Towels (bath, hand, washcloth)-1 set per guest plus 1 extra hand towel per guest. u. Room key card-1 per guest. v. Microwave and small refrigerator-1 per room. w. Iron and ironing board-1 per room. D. Facility Amenities: a. Washers and dryers, accessible 24 hours a day, two located per floor. b. Ample parking adjacent to building for one vehicle per room. c. One telephone per room. The following services must be performed/provided as part of the rental consideration: d. Insect prevention/extermination, as required, within three days of notification. e. Toilet Paper (as required by occupants). f. Maid Service at each change of occupancy and daily cleaning services consisting of the following: i. Change of bath linens. ii. Vacuum and sweep all floors covered with carpets; and scrub all tiled floors. iii. Clean mirrors, bathtubs/shower stalls, sinks, doorknobs, light switches, and toilets. iv. Dust furniture and decorations, and polish wood furniture. v. Remove trash. g. Maid service shall provide change of bed linens every three days. h. Twenty-four hour front desk attendant and/or security service. E. Local Amenities: Restaurant/fast food chain/ or government dining facility must be available within walking distance from the facility. F. Special Instructions: a. Each room will be occupied by one or two medical students. However, on occasion the individual occupant may have a guest. During these occasions the individual occupant must notify the front desk and pay any additional charges. If this is a double occupancy the front desk may require concurrence by the roommate. The Government will not be responsible for extra charges. b. Occupant of room will be responsible for the following facility rules and will be liable for any damages or loss of facility property, i.e., lost keys. The government will not be responsible or liable for the personal actions of individual occupants. c. Facility must meet standards required by the following laws and regulations: i. American with Disabilities Act. ii. Hotel Fire and Safety Act of 1990. d. The Government will provide the facility manager a schedule of student rotations. This schedule will depict the time periods the students will be using the rooms. During those periods or breaks wherein the room will not be occupied, the facility will make a reasonable effort to rent the room on a daily basis and will refund or rebate the appropriate amount to the government or deduct those costs from the monthly bills. G. The contractor shall submit monthly invoices to USUHS. USUHS will forward certified invoices to the financial institution office for payment. Contractor may choose to accept all payments by a VISA government purchase card. Submission of Quotes: All interested offerors shall submit their quotes for the base year plus four option years.
