- This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-05-T-0060 is issued as a request for quotation (RFQ). Clin 0001. Non-personal services for Survery Research Consultant for the period of August 01, 2005 through July 31, 2006 for 420 hours @ $____________ hr = $ _________. The duties to be performed shall be in accordance with the statement of work, (SOW). The services and duty hours will be performed at Naval Medical Center San Diego, Refractive Surgery Center, Branch Medical Clinic, Bldg 624-2, 2650 Stockton Road, San Diego, CA 92106, Survery Research Consultant, Ophthalmology Department PRK Program STATEMENT OF WORK, Survey Research Consultant 1. SCOPE OF CONTRACT a.The work to be performed is generated at the Naval Training Center and Naval Medical Center, San Diego. The contractor is a Research Consultant who provides data analysis on questionnaires in which she developed specifically for the refractive surgery projects to research normative data to use in up and coming study parameters for FDA/BUMED protocols in the on-going LASIK and Quality of Vision Metric programs. b.During the term of this contract, the contractor agrees to perform on behalf of the Government the duties as a Research consultant, in which the incumbent must aid in experimental design, statistics, and library skill to investigate and analyze data collected on previous questionnaires for extended studies on the PRK FDA/BUMED protocols. 2. DUTY HOURS a.The contractor shall be available for inquiry on any and all collected data as well as questionnaires developed and in the development stage, Monday through Friday 0800-1700. The contractor shall agree to extend these hours, as necessary to ensure deadlines are met. The contractor shall be compensated for these extended hours with compensatory time off at a latter date to be mutually agreed upon. The contractor will be available for meetings and engagements concerning any part of the development of the questionnaires past, present or future for the duration of this contract. 3. DUTIES a.Under the direction of the Research Director and Refractive Surgery Director the incumbent shall manage and conduct data analysis on pre/post operative data collected from questionnaire as well as formulating new questionnaires from data collected as part of the refractive surgery program. Manage computer data from both the visual performance testing and data generated by previous as well as future questionnaires. Conduct library searches for pertinent scientific literature, aid in the statistical analysis of data, prepare tables and graphs of results. Incumbent must be willing to travel at least 4 times out of the year. 4. REQUIREMENTS The contractor shall comply with the medical staff rules, regulations and bylaws related to clinical practice as promulgated by the MTF commander and the nationwide standards of practice of the refractive surgery research studies. The contractor shall become acquainted with and obey all station regulations, shall perform in a manner to preclude the waste of utilities, and shall not use Government telephones for personal business. All motor vehicles operated on DOD installations by the contractor shall be registered with the base security service according to applicable directives. Eating and smoking by the contractor are prohibited in patient care areas and are restricted to designated areas. All financial, statistical, personnel and technical data which is furnished, produced or otherwise available to the contractor during the performance of this contract are considered confidential business information and shall not be used for purposes other than performance of work under this contract nor be released by the contractor without prior written consent of the Contracting Officers Representative or his designated representative. Any presentation of any statistical or any analytical materials, or any reports based on information obtained from studies covered by this contract, will be subject to review and approval by the COR before publication or dissemination. The Secretary of the Navy has determined that the illegal possession or use of drugs and paraphernalia in a military setting contributes directly to military drug abuse and undermines command efforts to eliminate drug abuse among military personnel. The policy of the Department of the Navy (including the Marine Corps) is to deter and detect drug offenses on military installations. Measures taken to identify drug offenses on military installations and to prevent introduction of illegal drugs and paraphernalia include routine random inspection of personal possessions on entry or exit. If there is probable cause to believe that the contractor has been engaged in use, possession, or trafficking of drugs, the contractor may be detained for a limited period of time until he or she can be removed from the installation or turned over to the local law enforcement personnel having jurisdiction. When illegal drugs are discovered in the course of an inspection or search of a vehicle operated by a contractor, the contractor and vehicle may be detained for a reasonable period of time necessary to surrender the individual and vehicle to appropriate civil law enforcement personnel. Action may be taken to suspend working privileges as well as installation driving privileges. Implicit with the acceptance of this contract is the agreement by the contractor to comply with all Federal and State laws as well as regulations issued by the commander of The solicitation documet and incorporated provisions and clauses are those in effect through FAC 2005-04- Expires July 08,2005. Prior to Contract Award, the government reserves the right to review the resumes of contractor employees performing under the contract solely for the purpose of ascertaining their qualifications relative to the personnel qualifications terms of the contract. Accordingly, the contractor shall furnish such resumes and all professional requirements as stated in the Statement of Work (SOW) to the Purchasing Agent along with their quote.
