- This is for informatinal purposes only. The Centers for Disease Control and Prevention intends to award a purchase order to the American School Health Association (ASHA). The purpose of this purchase order is to obtain services that a vendor shall render in collaboration with CDC-Division of Adolescent and School Health to : 1) assist in the development of criteria and application materials, 2) identify and set up a web-based or hard-copy application process, 3) publicize and promote the program, 4) provide technical assistance to applicants and grantees, 4) coordinate the review process to select recipients, 5) assist in distribution of awards to schools, and 6) create a summary report of schools? experiences in implementing School Health Index action plans. The vendor shall provide all labor, materials, supplies, transportation, equipment, and perform services necessary to work with CDC-DASH staff to support the incentive program for School Health Index action plans. Specific Tasks include: Vendor or representative from the vendor?s firm or agency shall serve as a School Health Education Specialist/Developer to accomplish the following tasks as defined herein: (1) The vendor shall assist CDC-DASH in the development of criteria for selection of schools. (2)The vendor shall assist CDC-DASH in the development of application forms and requirements. (3) The vendor shall identify and set up either a web-based or hard-copy application process. (4) The vendor shall assist CDC-DASH in the development of the program announcement. (5) The vendor shall publicize and promote the program through listservs, journals, websites, newsletters, or other channels available. (6) The vendor shall provide assistance to applicants as needed. (7) The vendor shall coordinate the receipt of letters of intent and applications. (8) The vendor shall assist CDC-DASH in the identification of potential reviewers. (9) The vendor shall recruit and contact potential reviewers. (10) The vendor shall develop and send review materials and instructions to reviewers. (11) The vendor shall coordinate the receipt of the reviews and the selection of recipients. (12) The vendor shall send the notification of awards to grantees and assist in the distribution of funds to schools. (13) The vendor shall provide technical assistance to grantees as needed. (14) The vendor shall create a summary report of schools? experiences in implementing School Health Index action plans. (15) The vendor shall participate in meetings, conference calls, telephone and email communications, as needed, to discuss the program with CDC-DASH and other partners. This action is pursuant to 41 USC 253(1) and under the authority of FAR 6.302-1. The requirement will be procured as a commercial item in accordance with FAR Part 12.102(c) under the test program. A determination by the Government not to compete this proposed requirement based on responses to this notice is solely within the discretion of the Government. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The following provisions apply to this acquisition; 52.212-1 Instructions to Offerors - Commercials Items (MAR 2000) applies to this acquisition; 52.212-2, Evaluation - Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical capability of the proposed system to meet the requirements stated herein, and (2) price. The clause 52.212-4, Contract Terms & Conditions - Commercial Items (MAY 1999) applies to this acquisition. The following FAR Clauses are hereby added to this clause: 52.247-35 F.O.B. Destination; The clause at 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquisition: (1), (3), (5), (7), (9), (11), (12), (14), (15), (17), (18), (20) and (24). OFFERORS SHALL INCLUDE A COMPLETED COPY OF THE PROVISION AT 52.212-3, OFFERORS REPRESENTATIONS AND CERTIFICATIONS - COMMERCIAL ITEMS, WITH YOUR QUOTE. The best way to complete this certification is to download the FAR Clauses (Volume II) at: http://www.arnet.gov/far, print out the clause 52.212-3, Offeror Representation and Certification, and fill in the appropriate blanks. Quotes will be due August 1, 2005, by 8:30 AM EST at the Centers for Disease Control and Prevention; Acquisition and Assistance Branch B; Attn: Dorothy Reed Penix; 2920 Brandywine Road, Atlanta, GA 30341. All responsible sources that can meet the above requirement may submit a quote, which will be considered by the Agency. All vendors need to be registered in the Central Contracting Registry (CCR) prior to award of a Federal contract. The preferred method for completing registration is via the World Wide Web at http://www.ccr.gov. A handboook and other information are available at the website, including information on how to obtain a DUNS number, which is required to begin the registration process.
