- This is a combined synopsis solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Instructions To Offerors Proposal Submission 1. Offers will be received no later then Tuesday 09 August by close of business. The Period of Performance is 01 September 2005 through 30 June 2005 and subject to the availability of funds. 2. The following is to be submitted as part of your offer: a. Resume: A one page document, which identifies education, and 1 year of experience operating a program of community Information and Referral a ssistance within the Army Community Service ACS. b. Three references: References can be personal references or professional references family members are excluded. Reference information shall include name, address, telephone number, and an indication tha t the reference is personal or professional. The Government may contact these references with regard to past performance. c. The Contractor must be New Jersey based and within a fifty mile radius of Fort Monmouth. d. The contractor may be required to s ubmit to an interview if requested. PERFORMANCE WORK STATEMENT ARMY COMMUNITY SERVICE'S COMMUNITY INFORMATION AND REFERRAL PROGRAM C.1. GENERAL The purpose of the Community Information and Referral is to operate a program of community information and referral assistance within the Army Community Service ACS. ACS is a community oriented Social Service Program to assist eligible clients as set forth in Army Regulation AR 608-1 The Army Community Service Program hereafter referred to as AR 608-1. The contractor shall have successfully accomplished an ACS accreditation process per AR 608-1. The contractor shall have experience providing services to active duty and retired military affiliated families. C.1.1. SCOPE OF WORK The Community Information and Referral Program Contractor hereafter referred to as Contractor shall implement the Community Information and Referral Program to assist eligible clients as set forth in AR 608-1 and act as the primary point of contact for the orchestration and execution of all Community Information and Referral Program efforts whether or not he she is actually involved in the direct service delivery of each initiative. C.1.1.1. The Contractor shall provide Community Information and Referral for eligible clients as set forth in AR 608-1. The contractor shall provide the professional and administrative support and personnel to perform the service set forth in this Performance of W ork Statement PWS. This PWS consists of the following principle features: C.1.1.2. The contractor shall provide a program manager to work 37 hours each week, less 10 approved U.S. holidays and 10 days personal time, with prior approval of the COR. C.1.1.3. Services shall normally be 0800-1645, Monday through Friday, however, the nature of the service requires flexibility in work scheduling. Specific hours to be worked will be specified by the COR. C.1.1.4. Primary work site will be Fort Monmouth, NJ; secondary sites within a 200 mile radius, will be designated based on Army mission requirements. C.1.2. PERSONNEL C.1.2.1. The program manager shall have the following, documented, knowledge, skills, abilities and other qualities: C.1.2.1.1. Knowledge of methods and techniques of needs assessment and problem analysis. C.1.2.1.2. The Government will provide to the Program Manager for use in connection with this contract the following property: C.3.1.1. Building sufficient office space for one program manager. C.3.1.2. One class A telephone, to be shared with other staff. C.3.1.3. Heat, light, and other essential utilities. C.3.1.4. One computer for program implementation, as well as other equipment and supplies as needed and available. C.3.1.5. Standard U.S. Army and Government forms and regulations as required. C.3.1.6. Office furniture, to include desk, chair, filing cabinet, bookcase, and storage space. C-4 CONTRACTOR FURNISHED PROPERTY C.4.1. The contractor shall not be required to furnish any property or materials relevant to this contract, but shall be responsible for receiving, caring and accounting for and returning any government furnished property. The contractor shall familiarize him he rself with the proper care and use of government supplied property. The operation of any necessary equipment, to include training aides within the services to be delivered hereunder shall be at no additional expense to the government over and above the co ntract price. C.5. SPECIFIC TASKS C.5.1. The program manager shall have responsibility for administering and maintaining the Lending Closet, which makes available a variety of bas ic housekeeping items for temporary loan to eligible clients. C 5.1.1. The program manager shall have responsibility for administering and maintaining the Utility Deposit Waiver Program UDWP which is an agreement between Bell Atlantic, Jersey Central Power and Light, and New Jersey Natural Gas and the Commander, Fort Monmouth to assist military personnel assigned to Fort Monmouth. C.5.2. The program manager shall act as a Unit Service Coordinator to individual units as determined by COR. C.5.2.1. The program manager shall receive cross training in all other ACS program areas as determined by COR. C.5.2.2. The program manager shall provide program specific cross training to ACS contractors, volunteers and government employees as determined by COR. C.5.3. The program manager shall receive client inquiries, by telephone and inperson, concerning community services and interview clients to assess the type of assistance required. C.5.3.1. The program manager shall provide information ranging from limited factual response to more detailed factual information about community service systems, agency policies, and application procedures. C.5.4. Record keeping The program manager shall gather and maintain a variety of data as required by governing program guidance to support management information needs. C.5.4.1. The program manager shall gather, maintain, and analyze data for planning and reporting purposes on a monthly basis. C.5.4.1.1. The program manager shall provide a written report of data on a monthly basis. C.5.4.1.3. The program manager shall provide a written report of data on a yearly basis. C.5.4.2. The program manager shall develop, implement and maintain an SOP for the Lending Closet. C.5.4.3. The program manager shall develop, implement and maintain an SOP for the Utility Deposit Waiver Program UDWP. C.5.4.4. The program manager shall maintain a weekly record of significant activities, as defined by COR. C.5.4.5. The program manager shall record daily hours worked in implementation of contract. C.6. The program manager shall provide a individual participation file for each participant in the UDWP within 2 days of enrollment. C.7.2. The program manager shall provide a record of participation for each individual using the lending closet immediately upon use. C.7.3. The program manager shall provide the COR with a list of supplies and materials required for lending closet in January. C.7.4. The program manager shall provide an invoice of hours provide in implementation of contractor to COR by the 1st of each month. C.7.5. The program manager shall provide a Memorandum of Record within 2 working days to report an incident. C.7.6. The program manager shall provide a complete d DA FORM 5900-R per AR 608-1 to the COR by the 15th of next month. C.7.7. The program manager shall provide a weekly Significant Activities report by 12:00 p.m. on the following Monday. C.7.8. The program manager shall provide an SOP for each component of the program within 60 days of award of contract. C.8. TRANSPORTATION The contractor will supply his or her own transportation to perform the contract. Subject to the availability and at the authority of the Contracting Officer Representative, the contractor may use government vehicles if they obtain a government motor vehic le operators license. Contractors may apply and test for this license during the contract hours. M.1. Evaluation of Bids/Offers APR 1984 A bidder offeror must quote on all items in this solicitation to be eligible for award. All items will be awarded only as a unit. Evaluation of bids offers will be based, among other factors, upon the total price quoted for all items. M.1.1. A bidder offeror must provide a resume or documentation of eligibility specific to Performance Work Statement PWS SECTION C paragraph 1.2. M.2. Evaluation of Options JUL 1990 Except when it is determined in accordance with FAR 17.206 b not to be in the Governments best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement.
