Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 263-2005-P(GQ)-0055 is issued as an Request for Proposal (RFP). The solicitation/contract willl include all applicable provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-04. The NAICS code is 541611 and the small business size standard is $6.0. This requirement is set aside for small business. The requirement estimate quantity is 8 month. The contractor shall provide the following services: 1. Research the literature and compile an accurate complete historical account of NCMHD's success towards eliminating health disparities. 2. Conduct interviews with principals who will be able to contribute oral and written histories as provided by NCMHD. 3. Prepare a 150-page report in accordance with the objectives. 4. Conduct follow-up briefings to obtain the required clearances and approvals prior to publication. 5. Obtain photographs to support the project and use the generic photos of high quality to illustrate the activity as well as photographs and other illustrations as provided by NCMHD. 6. Work with NCMHD, to assemble the editorial, clear the text and coordinate all administrative requirements. 7. Provide design, graphic and desktop services, reproduction, and postal services to distribute report. 8. Provide laser proof for final approval to printing. 9. Distribute final product to national audience. 10. Shall provide the following deliverables: a. Four-color, 150-page, high quality publication. b. Printing and distribution nationally and internationally - 5,000 copies. c. Printing additional copies up to 1,000. d. Provide monthly progress reports, which include, by task, a brief statement of progress during the preceding month, problems encountered, proposed solutions, and initiatives undertaken. The report shall have an inventory attached that lists the name of the publication; total supply and the number of publications distributed that month. An accounting of costs by task, with a breakdown of labor hours and costs expended for each task during the reporting period and year to date. Two copies of the report shall be submitted to the Project Officer and one copy to the Contracting Officer. e. Provide a final report that includes a summation of the work performed and results obtained for the entire contract period of performance. This report shall be in sufficient detail to describe comprehensively the results achieved, including cumulative figures for quantifiable date. An inventory of report shall be included. The final report shall be submitted no later than the contract expiration date. Two copies of the report shall be submitted to the Project Officer and one copy to the Contracting Officer. FAR provision 52.212-1, Instructions to Offerors-Commercial applies to this acquisition. A completed copy of the provisions at 52.212-3, Offeror Representations and Certifications-Commercial Items, is required as part of the offer. The clause 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition. The clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. The contractor will be paid on a partial payment basis. The FBO Numbered Note: Note 22 apply. All firms must be registered in the Central Contractor Registration database (www.ccr.gov). Sources demonstrating the capability to accomplish the above shall provide pertinent information that is sufficient detail to demonstrate their ability to perform. Information furnished should include enough material to conduct a proper evaluation. Respondents will not be notified of the evaluation results. Offers are due on or before August 4, 2005 to Dorothy Nickens.
