Description
The VA National Acquisition Center will issue RFP 797-NC-05-0012 for the Department of Veterans Affairs_ (VA) requirements for Disposable Biohazard and Disposable Chemotherapy containers. Offerors will be required to provide containers that conform to the following specifications/descriptions: DISPOSAL CONTAINER, BIOHAZARD: Intended Use: For safe disposal of sharps. Product Criteria: Single Use_Dispose of when full; Puncture Resistant in compliance with any applicable current industry standard; Non-Removable Lid After Assembly; Molded Seams_Leak proof. Assorted Sizes to include at a minimum; Quart Size (approx), 2-Gallon Size (approx), 8-Gallon Size, (approx), 18-Gallon Size (approx); Unable to Retrieve Contents; Transparent "Fill" Line at 3/4 Full for Wall Unit; Horizontal Drop (wall mount) accommodates 60ml syringe and needle; Single Hand Operation; Ease of Correct Assembly; Container Security Device Available for Wall-Mounted Unit; Floor Models Opening Equal to, or Less Than 4" Diameter; Rigid Plastic; Tamper-resistant Mechanism for Final Disposal; Easily Visible International Biohazard Symbol and Signage. THIS PROCUREMENT HAS AN ESTIMATED ANNUAL VOLUME AS FOLLOWS: CHEMOTHERAPY - 2 Gallons 3,187 each, 5 Gallons 8 each, 8 Gallons 12,580 each, 12 Gallons 850 each, 18 Gallons 1,460 each and 1 Quart 200 each. BIOHAZARD - 10 Gallons 12 each, 11 Gallons 48 each, 12 Gallons 5,090 each, 16 Gallons 168 each, 18 Gallons 15,340 each, 19 Gallons 175 each, 30 Gallons 195 each, 0.6 Liter 100 each, 2 Liters 700 each, 21 Liters 920 each, 1 Pint 500 each, 0.7 Quart 648 each, 1 Quart 21,915 each, 1.5 Quarts 1,252 each, 1.7 Quarts 880 each, 2 Quarts 1,160 each, 2.2 Quarts 1,580 each, 2.5 Quarts 331 each, 3.2 Quarts 30 each, 4 Quarts 200 each, 5 Quarts 191,104 each, 5.4 Quarts 1,252 each, 6.9 Quarts 300 each, and 8.2 Quarts 134 each. The estimated quantities listed above are for VA only and are based on VA historical usage for sizes listed. The contract period will be for one year plus four one-year option periods. The contract items will be distributed through delivery orders to VA facilities. If offeror is not a manufacturer, a signed agreement between the offeror and the manufacturer must be submitted with offer and remain in effect for the duration of the contract period including option years. This procurement may be considered a bundled or consolidated procurement. Small business is invited to provide comments concerning this procurement to the Contracting Officer. The solicitation will be available for downloading, on or about the issue date of March 3, 2005 from the VA Office of Acquisition and Material Management_s (O&AMM) internet web site at http://www.va.gov/o&amm/busopp/sols.htm.
