Description
AMENDMENT 1 TO SOLICITATION 223-05-3000 SOLICITATION 223-05-3000 IS AMENDED AS FOLLOWS: 1. In Paragraph I. SCHEDULE OF PRICES, the dates are changed to read as follows: Base Year (March 1, 2005 – February 28, 2006) Option Year 1 (March 1, 2006—February 28, 2007) Option Year 2 (March 1, 2007 – February 29, 2008) 2. The following new Paragraph is added: VIII. PERFORMANCE REQUIREMENTS SUMMARY is added: Proposed Deductions for Not Meeting the Acceptable Quality Level In each of the Performance Requirements Summaries for this solicitation, each offeror shall propose the amount of deduction that they consider to be reasonable for the unacceptable performance of required services. The Government will subjectively evaluate these proposed deductions by comparing them to the offeror’s total contract price and as an indication of the offeror’s confidence and ability in satisfactorily meeting the performance standards. (see attached table) Performance Requirements Summary 1. Required Service: The successful offeror shall adhere to a timely mutually agreed upon disclosure policy. Standard: The mutually agreed upon turnaround time to be negotiated at the time of award. Acceptable Quality Level (AQL): Standard shall be fully met for each disclosure request. Method of Surveillance: Project officer and staff review of turnaround times. Deduction for Not Meeting the AQL: $ ___________ to be deducted per violation of disclosure policy request if standard if not fully met. 2. Required Service: The successful offeror shall deliver an acceptable projection or other appropriate methodology within 6 months after award. Standard: 6 months after contract award. Acceptable Quality Level (AQL): Standard shall be fully met. Method of Surveillance: Project officer and staff acceptance of projection or other appropriate methodology. Deduction for Not Meeting the AQL: $ ___________ to be deducted if projection or other appropriate methodology is not delivered within 6 months. 3. Required Service: The successful offeror shall guarantee timely access to patient-level analytical datasets. Standard: The mutually agreed upon dataset parameters (max. size, data elements) andturnaround time to be negotiated at the time of award. Acceptable Quality Level (AQL): Standards shall be fully met for access to all patient-analytical datasets. Method of Surveillance: Project officer and staff review of dataset quality and turnaround times. Deduction for Not Meetig the AQL: $ __________ to be deducted per violation of dataset standard if standard is not fully met. 4. Required Service: The successful offeror shall notify FDA about any circumstances resulting in loss of or lack of access to data from the database. Standard: Within one week of offeror's knowledge of the loss or lack of access to data from the database. Acceptable Quality Level (AQL): Standard shall be fully met. Method of Surveillance: Project officer and staff review of dataset quality (i.e. variance in data runs). Deduction for Not Meeting the AQL: $__________ will be deducted per violation of failure to notify FDA. 5. Required Service: The successful offeror shall notify FDA a minimum of 48 hours in advance of planned downtime of the database. Standard: 48 hours in advance of planned downtime. Acceptable Quality Level (AQL): Standard shall be fully met. Method of Surveillance: Project officer and staff monitoring of notification and database access times. Deduction for Not Meeting the AQL: $ ___________ will be deducted per violation of failure to notify FDA. 6. Required Service: The successful offeror shall notify FDA within 30 minutes from the onset of unplanned downtime with an estimate of time to restoration of access to the database. Standard: Within 30 minutes from the onset of unplanned downtime. Acceptable Quality Level (AQL): Standard shall be fully met. Method of Surveillance: Project officer and NOTE: THIS NOTICE MAY HAVE POSTED ON WWW.FEDBIZOPPS.GOV ON THE DATE INDICATED IN THE NOTICE ITSELF (06-JAN-2005).
