- This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation is available at request. The solicitation is issued as a Request for Quotation. The solicitation document and incorporated provisions, and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-27. The solicitation is 100% set-aside for Small Businesses. The associated NAICS code is 611710. The contractor shall provide on-site technical assistance and training. Services are needed for the school to meet the Consolidated School Reform Plans (CSRP) annual yearly-progress in reading, math, language arts, and other components as outlines in the CSRP. Contractor shall: 1. Conduct needs assessment and climate surveys. 2. Provide strategic planning and plan implementation and monitoring. 3. Provide data analysis and facilitation of Data Retreat activities. 4. Provide specific training workshops in a variety of content areas. 5. Provide guidance and training for the SIT leadership team. 6. Coach and mentor instructional staff. 7. Implement Parent Involvement; conduct parent training. 8. Implement Professional Development component. 9. Design data gathering system to monitor plan implementation and impact. 10. Assist with other reporting requirements and as needed. Contractor is required to provide statistics and data regarding any findings, improvements, recommendations, and success reports to the Principal, SIT Leadership Team, Parent Advisory Committee, all staff, and school board after each site visit; and a list of activities of meetings along with sign-in sheets of meetings. Services are needed at T?iis Nazbas Community School, Hwy 160, BIA Road N5114, Teec Nos Pos, AZ 86514. Period of Performance is August 2005 to July 31, 2006. The provisions 52.212-1 and 52.212-4 apply to this acquisition. The clause at 52.212-1 applies: The following factors shall be used to evaluate offers: (i) price; (ii) technical capability of the item offered to meet the Government requirement prices, and (iii) past performance. The clause at 52.212-5 applies to this acquisition and the following additional FAR clauses cited in this clause are applicable: (5) 52.219-6, (14) 52.222-3, (15) 52.222-19, and (32) 52.232-34. The following clauses are incorporated by reference: 1) 1452.226-70, Indian Preference; 2) 52.212-04; 3) 52.213-02; and 4) 52.246-04. Offerors shall submit a completed copy of the provision of 52.212-3 with its offer. A copy can be obtained at www.arnet.gov. Evaluation procedures: The Government shall award a firm-fixed price purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government.
