- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented, with the additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. For solicitation number RFQ 667-65-05, award will be made using the simplified acquisition procedures part 13.5. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. This acquisition is unrestricted. The North American Industry Classification System # 334510 and the small business are 500 employees. The acquisition is for the purchase of a Digital Video FEES System, (brand name), equal or better, and related equipment or service listed below. If or equal, supply adequate information to determine whether the equal can be used. The quoter shall provide pricing and warranty information on the following equipment/supplies: Xenon Light Source, Model 7150;Flexible Endoscope, Model 7105; Lens Adapter, Model 7116; Digital Video Recording Module, Model 7245; Color Printer, Model 7264C; Cart/Isolation Transformer, Model 7271; DVD+RW Media, Model 7202A; Scan Converter, Model 7139; Setup and Training, Model 7198; Shipping, if any. Delivery is to be made to Chief, A&MMS, Overton Brooks VA Medical Center, 510 East Stoner Avenue, Shreveport, LA 71101 4295. Delivery of equipment is desired between October 3, 2005 and November 30, 2005. Inspection and acceptance is at the Overton Brooks VA Medical Center, Shreveport, LA. Quote shall specify FOB, Overton Brooks VA Medical Center, Shreveport, LA (Destination). The provisions at FAR 52.212 1, Instructions of Offeror Commercial Items (Jan 2005) applies to this acquisition. There are addenda to these provisions. Add the following provisions: FAR 52.252-1, Solicitation Provisions Incorporated by Reference (Feb 1998), This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. Additional contract requirements: The vendor responding to this acquisition shall provide this office with the following information: a cost per item on company letterhead (equipment and consumables supplies), stating unit price, warranty, FOB, prompt payment terms, correct remittance address, and a completed copy of FAR 52.212 3 (Mar 2005), Offeror Representations and Certifications Commercial Items. If offering an equal product, the vendor must submit documentation of the product with the quote. For a company not in possession of FAR 52.212 3 dated Mar 2005, it has been provided as an attachment. Quotes must be in writing and accompanied by a completed Representations and Certifications (FAR 52.212-3) dated Mar 2005 or they will be rejected. Facsimile quotes will be accepted; oral quotes will not be accepted. This RFQ is subject to the availability of funds. Currently, funds are not available and the Government will not be under any obligation to purchase. Award will be made to the offeror whose quote conforms to the requirements of this solicitation, best meets the minimum needs of the Government and has the lowest price, all factors being considered. Award made be made without discussions.
