Description

This is a synopsis for a commercial acquisition using simplified acquisition procedures (Reference FAR Parts 12 and 13). THIS IS A SYNOPSIS AS REQUIRED BY FAR PART 5.2 ? THIS IS NOT A REQUEST FOR QUOTE/PROPOSAL. VENDORS SHOULD NOT SUBMIT FORMAL QUOTES/PROPOSALS UNTIL THE RFQ/RFP IS POSTED TO THIS WEBSITE. 45 CONS/LGCBB has an unfunded requirement for annual Mammography System Maintenance of a Lorad Mammography System Serial 22011000403 at 45th Medical Support Sq., Patrick AFB Florida. Service will start 1 October 2005 and contract will be awarded subject to availability of FY06 funds. Service provided will furnish all parts, glassware, labor, tools and transportation necessary to perform equipment maintenance, modifications, and software updates necessary to maintain the equipment. Period of performance/expected delivery date is 1 Oct 2005 to 30 September 2006. Four one-year options may be awarded. The government intends to make a single award to the lowest priced, technically acceptable offeror. This proposed procurement is a 100% small business set-aside. Solicitation No. FA2521-04-T-0326 is issued as a Request for Quotation. It is advertised with a Standard Industrial Code (SIC) of 7699, business size of 6 million and a North American Industry Classification System (NAICS) code of 811219. GSA Schedule holders for this item/service are encouraged to provide us with their GSA Schedule number. All responsible sources may submit a proposal, which shall be considered by the agency. The solicitation will be issued electronically on or about 18 August 2005 at www.eps.gov . Proposals may be submitted via hard copy, fax (321-494-5136), or e-mail.
