Description

A combined synopsis/solicitation Request for Quote (RFQ), Solicitation Number 583-00333-06, is issued in accordance with the Federal Acquisition Regulation (FAR) (http://www.arnet.gov/far/), FAR Subpart 12.6, Veterans Affairs Acquisition Regulation (http://vaww1.va.gov/oamm/vaar/), the following clauses and provisions. This announcement constitutes the solicitation for the commercial procurement to provide sodium chloride rock salt, type I water condition grade, in accordance with A-694B, DTD 6-22-83 and ANSI/AWWA, B-200-78 to the VA Medical Center, 1481 W. 10th Street, Indianapolis, IN 46202. The contractor shall deliver the sodium chloride rock salt with a hydraulic truck equipped with a twenty (20) foot hose (minimum). A written solicitation will NOT be issued. The Richard L. Roudebush VA Medical Center, Indianapolis, IN intends to award a Firm Fixed Price contract [IAW 52.216-1 Type of Contract (Apr 1984)]. The quote shall be submitted - By mail or Commercial Overnight type delivery to: Susan E. Hargett-Evans, (90CSC), Richard L. Roudebush VA Medical Center, 1481 Tenth Street, Indianapolis, IN 46202; Facsimile offers will not be accepted. Quote due date is September 8, 2005, by 4:30 p.m. local time. Contract period is 10/1/05 (or date of award) thru 9/30/06 with one (1) one year option renewal. The NAICS is 424690 and size standard is 500. All responsible sources may submit a quotation, which shall be considered by the government. Award will be based on pricing that is found to be fair and reasonable.
