Description
The USDA, Animal and Plant Health Inspection Service (APHIS), Plant Protection and Quarantine (PPQ), Emerald Ash Borer Project, Brighton, MI, intends to procure janitorial services for the period beginning October 1, 2005 through September 30, 2006. A firm fixed price purchase order will be awarded for the base year and one option year. Small business set aside under NAICS Code 561720, Business Size annual revenue $12 million. Submit faxed quotations to the Purchasing Section at (6122) 370-2136, Attention Joanne Mann. QUOTATIONS ARE DUE AUGUST 26, 2005 BY 10:00 A.M. local time. Simplified procurement procedures will be used as well as commercial item acquisition regulations per FAR 12. THIS IS A COMBINED SYNOPISIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation document incorporates provisions and clauses which are in effect through the current Federal Acquisition Circular. Applicable FAR clauses are incorporated by reference: 52.212-1 Instructions to Offerors ? commercial items; 52.212-2 Evaluation-Commercial Items; 52.212-3 Offeror Representations and Certifications; 52.212-4 Contract Terms and Conditions-Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, the following are considered checked: 52.222-21 Prohibition of Segregated Facilities; 52.222-26 Equal Opportunity; 52.222-35 Equal Opportunity for Special Disabled Veterans, Veterans of Vietnam Era, and Other Eligible Veterans; 52.222-36 Affirmative action for Workers with Disabilities; 52.222-37 Employment Reports on Special Disabled Veterans, Veterans of Vietnam Era, and Other Eligible Veterans; 52.222-41 Service Contract Act of 1965, as amended; 52.222-42 The Department of Labor Wage Determination 1994-2277, Revision 21, applies to this requirement; 52.232-30 Installment Payments on Commercial Items; 52.204-7 Payment by Electronic Funds Transfer-Central Contractor Registration. Price is approximately equal to past performance in importance. References are requested, see 52.212-1(10). ALL RESPONSIBLE SOURCES MAY SUBMIT A QUOTATION FOR CONSIDERATION. All offers shall be signed and include the Representations and Certifications 52.212-3, which may be submitted from the website: http://orca.bpn.gov. Offers shall include: Company Name, Company Address, Name of Contact Person, Telephone Number, Taxpayer Identification number, DUNS number. Quotes shall be submitted based on cost per month: Base Year ? Per Month $ ______________; One Option Year ? Per Month $ _____________. SPECIFICATIONS: Provide the following janitorial service five (5) days per week, Monday through Friday, before 7:00 a.m. or after 5:00 p.m., for an approximate 8,392 square foot facility. RESTROOMS AND SINK AREA: Floors shall be swept daily and wet mopped daily; Toilets or urinals shall be washed and sanitized and washbasins cleaned; Mirrors, shelving, dispensers, and chrome fixtures shall be damp wiped and sanitized; All other surfaces shall be spot-cleaned and horizontal surfaces dusted; Paper towel waste receptacles shall be emptied; towel, soap and toilet paper dispensers shall be serviced daily. OFFICE AREA AND CORRIDOR SPACE CLEANING: Wastebaskets shall be emptied and the wastepaper and trash removed to disposal area; Used plastic trash bags shall be replaced; Carpeted areas shall be vacuumed daily; Horizontal areas shall be dusted and spot cleaned weekly. Restroom cleaning and trash removal shall be performed once per day. Contractor shall supply all supplies required to service restrooms, trash bags and lines for trash containers.
