- This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is NHLBI -PS-2005-145-DDC, and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2001-27. The North American Industry Classification (NAICS) Code is 811219 and Sized Standard is $6.0 million for this requirement. This is not a small business set-aside requirement. This acquisition is being conducted under simplified acquisition procedures. The National Heart, Lung, and Blood Institute (NHLBI) intends to procure the service agreement coverage on a General Advanced Image Microscope (AIM-LSM, AIM-Pascal, and Cell Science). Service shall include the following: Scheduled and Emergency service performed between the hours of 8:00 a.m. and 5:00 p.m., Monday thru Friday, excluding Carl Zeiss MicroImaging, Inc. holidays; All travel charges within the continental U.S.A.; One (1) preventive maintenance inspection per Service Agreement per year. All replacement parts, except consumable items, applications support, and software upgrades when available, shall be performed by a factory trained service representative. Period of Performance: The proposed period of performance for this service will consist of one twelve (12) month basic contract period, and shall include options to renew for three (3) successive twelve (12) month periods. The provisions of FAR Clause 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The provisions of FAR Clause 52.212-2, Evaluation - Commercial Items is applicable to this acquisition. The proposals will be evaluated based on the technical requirement (1). Demonstrating the required performance to maintain the equipment. (2) Demonstrated availability, competence, and experience of the proposed technical personnel. (3) Documented successful past performance experience. The award will be made to the lowest priced, technically acceptable quote meeting the Government?s requirements. The Government intends to evaluate offers and award a contract without discussions with Offerors. Therefore, the initial offer should contain the offeror?s best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers and waive informalities and minor irregularities in offers received. The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The addenda to the clause reads as follows: The offeror must include in their quotation, the unit price, the list price, the prompt payment discount terms, the F.O.B. Point (Destination or Origin), the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. The FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items - Deviation For Simplified Acquisitions, applies to this acquisition. Interested vendors capable of furnishing the Government with the items specified in this synopsis should submit one (1) copy of the quotation to the below address.
