- US Naval Hospital, Okinawa, Japan intends to negotiate a sole-source standing order with Terumo Medical Corporation for the purchase of blood bags. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors ? Commercial Items; 52.212-2 Evaluation ? Commercial Items; 52.212-3 Offeror Representations and Certifications ? Commercial Items; 52.212-4 Contract Terms and Conditions ? Commercial Items; and 52.212-5 Contract Terms and Conditions to Implement Statutes or Executive Orders ? Commercial Items. NAICS for this requirement is 339112. The proposed procurement is unrestricted. Requirements are to be delivered beginning Oct 1, 2005 through September 20, 2006 and include the following item: CPD/OPTISOL QUADRUPLE BLOOD BAG WITH 16GA ULTRA THINWALL DONOR NEEDLE, 450ML XT PRIMARY CONTAINER WITH ONE 500ML XT612 PLUS 5-DAY PLATELET BAG AND TWO 400ML SATELLITE BAGS: 1BB*AGQ456A4; 24EA/CASE FOR TOTAL OF 156 CASES TO BE DELIVERED AS 13 CASES MONTHLY. The contractor shall be responsible for delivery to Tracy CA, 95304. This notice of intent is not a request for competitive quotes; however, any firm that believes it can meet the requirements may provide written notification to the Contracting Officer within 7 calendar days from the date of publication of this synopsis. Supporting evidence must be furnished in sufficient detail to demonstrate the contractor?s capability to provide and ability to comply with the above requirements. Information can be e-mailed to the Point of Contact. The Government will award a contract to the responsible offeror whose offer will be most advantageous to the Government, price and other factors considered. The following factors will be used to evaluate offers: 1) Technical capability of the item offered to meet the Government requirement (including submission of descriptive literature); 2) Price; all to be submitted with the quotation. Contract effective date estimated to be October 1, 2005 for a period of one year ending September 30, 2006. Contract award is subject to availability of funds. No telephone requests will be accepted. Responses received will be evaluated, however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, US Naval Hospital, Okinawa, Japan will proceed with the sole source procurement with Terumo Medical Corporation. Faxed offers will be accepted. Offerors are responsible for obtaining any and all amendments or additional information. Quotes and all pertinent data (descriptive literature, specifications, etc) must be submitted to the POC no later than August 26, 2005.
