- Ammendment to change required delivery date from 15 Sep 05 to 15 Oct 05. ****** MATTRESSES, One Hundred Seventy-Nine (179) Each (1) This is a combined Synopsis/Solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. (2) Purchase Request F1W3E35167A100 IS issued as a Request for Quotation (RFQ). (3) The solicitation document and incorporated provisions and clauses are those in effect through FAR Circular 2005-04. (4) The solicitation is set aside 100% for Small Business. The NAICS Code is 337910; Size Standard: 500 employees. (5) DESCRIPTION: Mattresses must be TWIN EXTRA-LONG 38” x 80” and have the following characteristics: anti-microbial/fungal/bacterial features and be hypoallergenic; impervious to bed bugs; meet fire code CTB 603, CTB 129, and Boston IX-11; have 6 double wire edge sppports, lock stitched tape edges, and integraded construction of water repellant cover (not a slip-on/zip-on cover). Contractor shall be responsible for delivery. (6) Specifications are listed in item 5. (7) DELIVERY FOB: 7079 Glacier Hall, chenault Ave, ELMENDORF AFB, ALASKA 99506. Requested delivery 15 Oct 2005. The delivery will be coordinated with SSgt Aaron Nitz 907-552-1774 or MSgt Robert Taylor at 907-552-9949. (8) FAR 52.212-1-Instruction to Offerors, applies to this solicitation. (9) This solicitation incorporates 52.212-2, Evaluation of Commercial Items. Award will be base upon lowest price/technically acceptable offer. The government reserves the right to make award on the initial quote received without discussion. (10) Each offeror shall include a completed copy of the provision at FAR 52.212-3 Alt 1, Offeror Representations and Certifications—Commercial Items with Alternate 1 with the offer. The Representations and Certification can be completed at http://orca.bpn.gov. (11) FAR 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition.
