- This is a combined synopsis solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation: proposals are being requested and a written solicitation will not be issued. Solicitation RFQ 548-05-344 This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-05 effective July 27, 2005. This solicitation is full and open competition. The associated NAICS # 621999 with a size standard of $8.5 million. The VA Medical Center, West Palm Beach, Florida, has a requirement for Diabetic Retinopathy Detection, Staging and Monitoring Services. The contract period will be from September 1, 2005 through December 31, 2005. WPB VAMC estimates approximately 1,200 patients for the life of the contract. Contractor shall provide pricing on a per patient basis (1200 each), to be all inclusive of photographer, equipment, Doctor, interpretation and written reports. The contractor shall provide: 1. Equipment, Photography Technician and Off-Site Optometrist or Ophthalmologist for image reading. Contractor shall provide imaging system that includes photography workstations with computer and digital equipment. This includes fundus camera, digital camera back, and stereo emitter and glasses. The computer system will include monitor, computer, UPS and Contractor?s proprietary image and data transmission software. Included shall be all maintenance PM?s (with parts & labor) and calibrations necessary for operation. If the equipment is damaged, a field engineer will be dispatched from the Contractor?s office to service the equipment and return it to the original delivered configuration. The contractor will be required to provide backup equipment if equipment is down for greater than 48 hours. Photography Technician: The Contractor will supervise the photographer. Photographer shall not be deemed or construed as a VA Employee. The Contractor is responsible for the quality of images performed by the photographer. The Contractor will submit to VA Contracting Officer, quality measures, review findings, and corrective measures taken of all work performed. Professional appearance of photographer is required at all times. Contractor shall provide backup for technician if he/she requests sick/annual/family leave to ensure no interruptions of scheduled patients. 2) Interpretation of the images and written reports: a. Contractor shall provide medical supervision of the program, b. Image evaluation services for the reading of the images. All interpretations of the images shall be done by a credentialed and licensed Optometrist or Ophthalmologist. Optometrist or Ophthalmologist shall be credentialed and privileged by the WPB VAMC. c. Contractor shall provide an electronic report back to the referring physician through CPRS Electronic Medical Record. Report is required to be available in CPRS within 48 hours of photograph. d. Contractor shall provide direct physician-to-physician contact by telephone should a patient need immediate attention. All results will be entered into CPRS. e. Documentation to include, but not be limited to, ETDRS Final Retinopathy Severity Scale Level for each eye, macular edema stage for each eye, patient care recommendation according to the Management of Diabetes Mellitus. Washington, DC: VA/DoD Clinical Practice Guideline Working Group, Veterans Health Administration, Department of Veterans Affairs and Health Affairs, Department of Defense, December 1999 (Update 2003), Office of Quality and Performance publication 10Q-CPG/DM-03. f. Provide ETDRS field 1 and 2 images from each eye of patients recommended for referral to an ophthalmologist to VAMC on a weekly basis (CD-ROM or DVD). g. Documentation will also include necessary information to meet clinical reminder requirements for retinal screening. h. Monthly report to the VA Medical Center listing the total number of services performed and detailed information regarding each individual Service, specifically including: (1) date of service; (2) Patient Unique Identifiers; (3) ETDRS Level; (4) ETDRS Macular Edema Status; and (5) Management Recommendations. i. Provide VAMC with documentation of established quality assurance protocols. j. All images and other medical record data will be labeled and stored consistent with standards for electronic patient medical records, HIPPA and VAMC policies and procedures. West Palm Beach VA Medical Center will provide the following: a. Adequate space for the camera system or systems. b. IT Access and Support c. Access to VA Computerized Patient Records System (CPRS) for review of medical information and entry of report. d. Remote access (VPN) e. Pharmaceutical supplies for dilation and topical anesthesia f. Disposable sunglasses for patient use following pupil dilation. g. Primary Care Referrals and list of patients eligible for the imaging service. Work Schedule: a. The Contractor shall be required to furnish services during normal VA business hours (8:00 am - 4:30 pm), excluding (Federal) national holidays. National (Federal) holidays are: New Year?s Day, Martin Luther King Jr. Birthday, President?s Day, Memorial Day, 4th of July, Labor Day, Columbus Day, Veterans? Day, Thanksgiving Day, Christmas Day. When one of the holidays falls on Sunday, the following Monday will be observed as a legal holiday. When a holiday falls on a Saturday, the proceeding Friday is observed as a national holiday by U.S. Government Agencies. Also included would be any day specifically declared by the President of the United States of America as a national holiday. The provision at 52.212-1, Instruction to Offerors - Commercial Items, applies to this acquisition. The provision at 52.212-2 Evaluation ? Commercial Items. The following evaluation criteria shall be included in Paragraph a of that provision: Evaluation Criteria for Diabetic Retinopathy Detection/Screening Services: 1) Number of years providing these services 2) Contractor shall provide certification or proof of training for Photographer/Technician 3) Contractor shall provide license of optometrist or ophthalmologist who will be reading images 4) Type of equipment to be provided and age 5) Contingency plan for equipment downtime 6) Contingency plan for scheduled and unscheduled leave of contractor employees 7) Contractor shall provide start up plan for services to begin on 9/1 8) Contractor shall provide services in accordance with National Institute of Health (NIH) Gold Standard Guidelines for Evaluation Detection, Monitoring, and Staging for Diabetic Retinopathy. Award will be made based on best value (ie: technical capability, past performance and price). Include a completed copy of the provision at 52.212-3, Offerors Representations and Certifications ? Commercial Items, with your quote. The clause 52.212-4, Contract Terms and Conditions ? Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? Full text of above referenced provisions and clauses may be obtained through the following websites: http://www.arnet.gov/far/ and www1.va.gov/oamm/vaar/index.htm CONTRACTOR PERSONNEL SECURITY REQUIREMENTS: a) Contractor shall insure the confidentiality of all patient and employee information and shall be held liable in the event of breach of confidentiality. Any person, who knowingly or willingly discloses confidential information from the VA Medical Center, may be subject to fines of up to $20,000.00. b) All Contractor employees who require access to the Department of Veterans Affairs' computer system shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. A minimum background check will cover a five year period comprised of a review of National Agency Check (NAC) records; a credit report; an interview with the subject, current supervisor and a verification of education degree. All costs associated with obtaining clearances for Contractor provided personnel will be the responsibility of the Contractor. Further, the Contractor will be responsible for the actions of all individuals provided to work for the VA under this contract. In the event that damages arise from work performed by Contractor provided personnel, under the auspices of this contract, the Contractor will be responsible for all resources necessary to remedy the incident, This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start date of the contract, the Contractor will be responsible for the actions of those individuals they provide to perform work for VA. Should a vendor other than OPM or the Defense Security Service (DSS) conduct the investigation, the investigating agency must possess OPM and DSS certification. Further, the investigative/clearance history must be maintained in the databases of OPM, the Defense Central Investigations Index (DCII), or the Defense Industrial Security Clearance Organization (DISCO) 1) Position Sensitivity - The position sensitivity has been designated as low risk. 2) Background Investigation - The level of background investigation commensurate with the required level of access is National Agency Check and Written Inquires. 1) Contractor Responsibilities: The Contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM), the Contractor shall reimburse VA within thirty (30) days. A) The Contractor shall prescreen all personnel requiring access to the computer systems to ensure they are able to read, write, speak and understand the English language. B) The Contractor shall submit or have their employees submit the following required forms to the VA Office of Security and Law Enforcement within thirty (30) days of receipt. Standard Form 85P, Questionnaire for Public Trust Positions, Standard Form 85P-S, Supplemental Questionnaire for Selected Positions FD 258, U.S. Department of Justice Fingerprint Applicant Chart, VA Form 0710, Authority for Release of Information Form Optional Form 306, Declaration for Federal Employment, Optional Form 612, Optional Application for Federal Employment. The Contractor, when notified of an unfavorable determination by the Government, shall withdraw the employee from consideration from working under the contract. Failure to comply with the Contractor personnel security requirements may result in termination of the contract for default. In any event, the Contractor shall provide individual copies of background investigations performed by the Contractor, to the VA, for each employee assigned to work under this contract as well as employee name, address and social security number. Government Responsibilities: The VA Office of Security and Law Enforcement will provide the necessary forms to the Contracting Officer after receiving a list of names and addresses from Contracting Officer. Upon receipt, the VA Office of Security and Law Enforcement will review the completed forms for accuracy and forward the forms to OPM to conduct the background investigation. The VA facility will pay for investigations conducted by the Office of Personnel Management (OPM) in advance. In these instances, the Contractor will reimburse the VA facility within 30 days. The VA Office of Security and Law Enforcement will notify the Contracting Officer and Contractor after adjudicating the results of the background investigations received from OPM. The Contracting Officer will ensure that the Contractor provides evidence that investigations have been completed or are in the process of being requested. Quotes are due @ 2:00 pm local time on August 23, 2005 at the VA Medical Center 7305 N. Military Trail West Palm Beach, Florida 33410.
