- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation FA4800-05-T-0006 is issued as a Commercial Solicitation with Request for Quotation (RFQ) using Simplified Acquisition Procedures found in FAR Part 13. This solicitation document and incorporated provisions and clauses are those that are in effect through Federal Acquisition Circular 2001-26. The NAICS code is 713940 -- Fitness and Recreational Sports Centers, NTE $6,000,000.00. This acquisition is a 100% set aside for Small Business. The 1st Contracting Squadron, Langley AFB, VA has a requirement for the following items: CLIN 0001 Aerobics Instructor. Quantity: 1, Unit: Each. Requirement includes four one-year option periods beginning with CLIN 1001 through 1004. Description of requirement: Provide aerobics instruction and conduct an aerobics program for the period beginning at contract award through expiration date shown on contract in accordance with the attached Performance Work Statement (PWS). Period of Performance: Base year of performance will be from 1 Oct 2005 through 30 Sep 2006, with four option periods. Place of performance will be Shellbank Fitness Center, and ACC Campus Fitness Center, Langley AFB, VA 23665. The provision at FAR 52.212-1, Instructions to Offerors Commercial applies. The Government will award a firm-fixed price Purchase Order resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, represents the best value to the government in terms of technical acceptability and price. The contractor shall submit at least three reference sources (at minimum references should include Point of Contact and Alternate, organization name, phone number, address, a description of services performed, and length and date of performance), a copy of a sample monthly schedule detailing type of aerobics instruction and proposed times, and a copy of all required certifications in accordance with the PWS. Offerors are reminded to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items with its offer. Clause 52.212-4, Contract Terms and Conditions Commercial Items, applies and is incorporated by reference.
