Description
Provide all labor, materials, equipment and supervision necessary to paint all previously painted stairwells in Main Hospital and the Outpatient clinic of the hospital. The stairwells consist of two (2) eight story (east and west) One (1)- nine (9) story (center), and one (1) 4-1/2 story North) stairwells with in the main hospital. This will include five (5) two story stairwells in the outpatient clinic. All painting will include handrails, face of risers, treads supports, sprinkler systems piping, walls, and ceiling with latex semi gloss paint in each stairwell. Stairwells will not be closed during painting to retain emergency egress. However as part of temp signage provided by the contractor, each stairwell door shall have a sign, notifying occupants that painting is in progress and that the stairwell is to be used for emergencies only. For Infection control this job is Type 1, Low Risk - which makes it Class 1 work per the attached matrix. Contractor shall commence all work within fifteen (15) calendar days of award and complete all work within (90) calendar days. Per FAR 52.219-3 NOTICE OF TOTAL HUBZONE SET-ASIDE (JAN 1999), this procurement is a HUB Zone set-aside. Any award resulting from this solicitation will be made to a HUB Zone small business concern. Accordingly, offers or quotations under this acquisition are solicited from HUB Zone small business concerns only. Offers or quotations that are not from a HUB Zone small business concern shall not be considered and shall be rejected. Per FAC 5252.228-9305 NOTICE OF BONDING REQUIREMENTS (DEC 2000), within 15 days after receipt of award, the offeror to whom the award is made shall furnish the a Payment Bond (Standard Form 25A). The payment bond shall be in a penal sum equal to 100% of the contract price. As the clauses at 52.236-2, Differing Site Conditions, and 52.236-3, Site Investigations and Conditions Affecting the Work, will be included in any contract awarded as a result of this solicitation, you are urged and expected to inspect the site where the work will be performed. A non-mandatory site visit is scheduled for 10:00 AM on Tuesday 23 August 2005. We will meet at the East or outpatient entrance to the Naval Hospital at that time, and then proceed to the project locations. Contractors are advised submit all technical questions they may have regarding this project in writing addressed to the Contracting Officer. Quotes are due by the close of business on 07 Sept. 2005. Contractor?s will need to submit the following information: 1) SF-18 Completed/Signed Page 1 (Include DUNS Number in Block 13) 2) Lump Sum Price Quote Page 2 (Firm Fixed-Price) 3) Section K - FAR 52.204-8 ANNUAL REPRESENTATIONS AND CERTIFICATIONS (JAN 2005) Pages 15-17 This solicitation will remain as written unless it is amended in writing. Do not consider verbal discussions or agreements by any representative of the Government as modifying the solicitation, unless they are reflected in a written amendment. You must acknowledge receipt of all amendments on your quote submission.
