Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes ar e being requested and a written solicitation will not be issued. The solicitation number is W91YTV-05-T-0206 and is a request for quotation (RFQ) and Notice of Intent to make an award. This is a total Small Business Set-Aside. The Standard Industrial Cla ssification Code is 8742 and the small business size standard is $6,000,000. The North American Industry Classification System Code is 541611. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. This procurement will be processed in accordance with Parts 12 and 13 of the FAR or some combination thereof. Womack Army Medical Center, Fort Bragg, NC, has a requirement for a Role Competency Workshop. The requested educational program Rebuilding a Professional Practice Model: A Strategic Role for Nursing Leadership is copyrighted and proprietary to Dr. Maria W. ORourke. The Government intends to make a Sole Source award to Dr. ORourke for five training sessions in October 2005. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial, apply to this acquisition. Addenda to FAR 52.212-1: Paragraph (f) of Far clause 52.212-1, Instructions to Offerors, does not apply to Request for Quotations (RFQ). STATEMENT OF WORK  WOMACK ARMY MEDICAL CENTER OROURKE PROFESSIONAL ROLE DEVELOPMENT & ROLE SOCIALIZATION TRAINING 1. GENERAL 1.1. DESCRITION OF WORK 1.1.1. Scope. The contractor shall provide a basic education program workshop for clinical leadership team at Womack Army Medical Center (WAMC) and outlying clinics. The contractor purpose is to provide the clinical leadership team with opportunity to incr ease their own understanding of core content related to the professional Registered Nurses (RN) role and its role accountability in order to create a consistent understanding of role and standards based practice and guide professional RN workforce developm ent. 1.1.2. The Deputy Commander for Patient Service and Assistant Chief Nurse will evaluate the quality of service rendered. 1.3. PERFORMANCE MEASURE 1.3.1. Security 1.3.1.1. Security Identification Badges. Contractor/Contract employees shall comply with the local installation and MTF personnel identification and access requirements. 1.3.1.2. Contractor/Contract employees shall turn in the MTF badge and Civilian ID Card to the Contracting Officer Representative (COR) or his designated representative upon termination of their service under this contract. 1.3.2. Vehicle Registration 1.3.2.1. Fort Bragg is a restricted access military installation. All contractors/contract employees must register their vehicles with the Provost Marshals Office to gain access onto the military installation. A valid drivers license, government-furnishe d civilian ID, proof on insurance and current registration must be presented to the Provost Marshals Office, at which time a Department of Defense decal will be issued. The decal shall be placed on the vehicles front windshield in accordance with instruc tions. 1.3.2.2. All vehicles, with or without a DOD decal, are subject to search. Contract employees may encounter long delays for vehicle inspection and identification checks upon entering and exiting the installation. 1.3.2.3. Contract employees shall follow installation procedures for removal and turn-in of the vehicle decal upon termination of services under this contract. 1.3.2.4. Expected Outcome. The government requires the contractor to provide a quality basic education program workshop that will approach key leadership roles in the organization, the appropriate use of the professional RN role in the care delivery model, and increase understanding how to meet the management obligation to uphold role and standards based practice (Maria W. ORourke, 2005) 1.4. TECHINICAL REQUIREMENTS 1.4.1. SPECIFIC TASK/RESPONSIBILITIES 1.4.1.1. Provide Basic Education Program Workshop. The contractor will use The Professional Role Development, Role Socialization Training Program and the leadership management version Program M that includes both the basic program (Program-MBV) and the adv anced program (Program MAV). Government will not accept any other program than the above listed. 1.5. PERIOD OF PERFOMANCE 1.5.1. Hours are between 0700-1630. Hours may change at discretion of the Deputy Commander for Patient Services or Contractor. 1.6. CONTRACTOR FURNISHED SUPPPLIES/SERVICE 1.6.1. Services 1.6.1.1 Copy ready program materials, which will be duplicated by the MTF for distribution to the participants. 1.7. GOVERNMENT FURNISHED SERVICES/SUPPLIES. 1.7.1. The MTF will provide on-site staff and management leadership support necessary to carry out Program-M for this contract and includes any necessary clerical and technical staff assistance and/or professional staff assistance.
