- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E050043, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 624310 and the business size maximum is $5.0 Million. The proposed contract is set aside for small businesses. All responsible small business sources may submit a quote, which will be considered by the Government. This office is requesting quotes on the following item: 0001: Provide all travel, supplies, materials, and personnel to provide Psycho-Educational Assessment/Evaluation Services to the BIA, Kayenta Community School, Kayenta, AZ in accordance with the Scope of Work (SOW). There shall be 18 Initial Evaluations, 10 Re-Evaluations, and Consultation Services shall be provided 6 hours per day for 18 days/visits. The QUANTITY is One (1), The UNIT is Lump Sum. Please provide a breakdown of your costs, which shall include at a minimum, cost per evaluation (Initial and re-evaluations), hourly rate and travel costs (mileage and lodging). Please provide a copy of your credentials. The Government intends to award a firm fixed price contract. The period of performance is from Date of Award (not later than August 22, 2005) through June 30, 2006. The SCOPE OF WORK (SOW) is as follows: Scope of Work. SCOPE: Kayenta community School, North Highway 163, Kayenta, AZ is in need of a contractor to provide Psycho-Educational Assessment Services for students who are identified as needing service. Students receiving this service are those requiring 3-year re-evaluations or newly referred students who may be disabled. The Psycho-Education Assessment Services will be in accordance with P.L. 105-17 and 34CFR, part B, 25 CFR, #1-80 and Section 504 of the Rehabilitation Act of 1973, 29 US.C 794. The period of service will be from the time of contract approval through June 30, 2006. The individual psycho-educational assessment service will include: The administration of a battery of tests chosen in collaboration with the Special Education Department; 1. The scoring of all tests administered. 2. The interpretation of the test results; 3. The provision of a comprehensive written report describing the results of the assessment to the school within 30 days of the evaluation; 4. Documentation that parents and staff receive a copy of the individualized report; 5. Availability of the psychologist to participate in MDET/MDT/IEP meetings where the results of the testing are interpreted to parents/guardians and staff within 60 days of the evaluation; 6. Follow the Arizona Evaluation requirement format. The individualized written psycho-educational report will include: 1. Basic student data as furnished by the referral packet at the Kayenta Community School Special Education Office; 2. Diagnostic statement categories listed in 34 CFR; 3. Reason for referral 4. Background information: educational status and history, health assessment, and prior evaluation history; 5. Behavioral observations; 6. Assessments as requested on permission to evaluate which may include: a. intelligence b. academic achievement c. social behavior d. perceptual abilities e. emotional behavior f. ADHD g. Other assessments relevant to the performance of the individual child 7. Summary report 8. Recommendations for placement, instruction, social, medical, etc. The contractor shall: 1. Be available five days after the effective date of the contract award through June 30, 2006 for six (6) hours per day. (This will average 2 days per month, however, due to scheduling conflicts, site assessments, school breaks, etc. Scheduling monthly site visits may not be feasible). The school psychologist will arrange to meet with the MDT and IEP Committee for their annual review, re-evaluations, and other concerns. 2. Follow the Arizona Evaluation requirements as stated in section 300.352, Evaluation and Procedures; and 3. Attend all school IEP/MDT meetings to review documents and provide professional input concerning the service delivery for the student. 4. Attend the Western Navajo Agency, Special Education Provider?s meeting (schedule date for November 2005). 5. Provide their current state certification to both Western Navajo Agency-Special Education and Kayenta Community School Special Education Department. The contractor will not be allowed to add service days and additional lodging without the modification approval from the contracting officer. The Offeror shall provide the lump sum cost for the base and four option year periods with a breakdown of their cost to provide this service and provide a copy of their certification. The offeror shall have a favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program prior to contract award (not later than August 22, 2005).
