DESIGN-BUILD SOURCE SELECTION FOR INDIAN HEALTH SERVICE, YOUTH TRANSITIONAL TREATMENT CENTER, WADSWORTH, NEVADA: SOLICATION HHSI-161200500020C. DESCRIPTION OF WORK: The intent of this project is for the planning, design and construction of a new fully functional 18,000 Sq. Ft. treatment center including all spaces with 16 bed patient living area, recreation and social space, public and support space and treatment area. Design shall include, site planning and drainage engineering, roads, walks, utilities and other infrastructure civil engineering, landscape design, architectural building design, structural engineering for buildings and other structures, mechanical engineering for building plumbing and HVAC systems, electrical engineering for building systems and site lighting, interior design, and any and all other architectural and engineering tasks as required for a complete project. Construction contractor shall be responsible for satisfactory construction of the project in complete compliance with all plans and specifications developed. Pricing will be designing within fee limitations and the construction will be negotiated lump sum total price. Project funding may be incremental. Specific requirements will be furnished as Phase II of the solicitation requirements. The project site is on the Pyramid Lake Indian Reservation, in Wadsworth Nevada. The estimated project size is in excess of $1 million dollars for both design and construction. Bid, payment and performance bonds are not required at this time. Bid bonds will be a requirement of the Phase II submittal package. Payment and performance bonds will be a requirement prior to construction phase. NAICS CODES 236220 SIZE STANDARDS IS $27.5 Million for each. Phase I of this solicitation will be a competitive formal Source Selection process in accordance with Part 15 of the Federal Acquisition Regulation (FAR) to ensure selection of the source evidencing the best overall capability to perform the work in a manner most advantageous to the Government, as determined by evaluation of proposals in accordance with the established criteria. The most highly qualified firms selected will be asked to participate in Phase II of this procurement for conceptual design and to prepare a detailed cost proposal for further consideration by the Government. The qualifications submitted by each firm will be reviewed and evaluated to identify the most highly qualified firms. When responding to this announcement, firms should fully address their capability and qualifications with regard to each of the following evaluation factors. Firms should submit their complete qualifications as they may be evaluated solely on their written responses to this announcement. Oral interviews may be held at the option of the Government. Prime Contractors assembling a team are cautioned that that the evaluation will be based on the team, not just the prime firm. The Offerors must demonstrate its experience and professional ability in the following areas and must submit all of the following information: All factors are equal. (1) PROFESSIONAL QUALIFICATIONS AND EXPERIENCE OF FIRM/TEAM AND ALL CONSULTANTS-Detailed Design Build experience; experience of Registered Architect and construction firm. Offeror must show design and construction experience on similar projects in a hospital environment of similar size and scope. Elaborate on projects that have been completed in the last 3 years. Provide references that may be contacted. The Registered Architect for the project and all consultant firms must submit a SF-330 Architect-Engineer Qualifications with supplement forms as need to be considered. (These forms may be obtained at gsa.gov.) (2) PROFESSIONAL QUALIFICATIONS AND EXPERIENCE OF THE PROPOSED STAFF FOR THIS PROJECT: Professional qualifications of project managers, superintendents, estimator, quality control, and construction inspectors including professional registration, education, training, technical competence and skills as they relate specifically to medical projects. Individuals submitted for evaluation may not be switched and must be used in their capacity on the project. (3) MANAGEMENT PLAN: Proposed project approach, construction method to accomplish contract requirements including teaming planning between contractor/sub-contractors, the designer of record and Government representatives. Cost management; Critical path scheduling; Value engineering; Fast track experience must also be presented for consideration. (4) QUALITY CONTROL/QUALITY ASSURANCE/ SAFETY EXPERIENCE DURING CONSTRUCTION PHASE: Description of specific safety and quality control initiatives and plans for this project. Provide information on your firms QA/QC procedures to ensure plans and specification is complete and accurate, including coordination of drawings between disciplines. Show how firm ensures applicable codes and standards are met in the design. The construction contractor must provide a copy of the Contractors EMR (Experience Modifier Rate) for the last 3 years. (The EMR is a reflection of there safety performance as compared to the average contractor in their specific specialty and is calculated over a three year period. An EMR of 1.00 is average. This rating is used to determine the worker's compensation insurance premium) (This will also be a requirement for all sub-contractors when identified). (5) PAST PERFORMANCE WITH GOVERNMENT AND PRIVATE INDUSTRY CONTRACTS OF SIMILAR SIZE, SCOPE, AND COMPLEXITY: Past performance on contracts with Government entities including Native American organizations, tribal governments, federal agencies, i.e., Indian Health Service and Bureau of Indian Affairs and Private Industry. In accordance with FAR 15.305(a)(2)(ii), the Offerors is required to identify past or current contracts (including Federal, State, and local government and private enterprise) for efforts similar to the current Government requirements in this solicitation. The Government shall consider this information as well as information obtained from other sources when evaluating the Offerors past performance. The Offerors are reminded that the Contracting Officer shall determine the relevance of similar past performance information. The Offerors shall limit this information to not more than 10 contracts but no less than 3 similar contracts, performed within the last 3 years. Failure of an Offeror to provide a minimum of 5 relevant references or the inability of the Government to reach at least 3 references, after making a reasonable effort to do so, may result in the Offeror not being rated on the past performance factor. An Offeror who has no relevant experience will be given the mid-point rating of the maximum possible score allowed for past performance. Mandatory Submittal Requirement: Past Performance information must include the following information and be submitted in the following format and order: 1. Contract Number; 2. Contract Award Date; 3. Contract Type (e.g., fixed price, cost reimbursable, etc.); 4. Contract Term (e.g., one year, base year and option years); 5. Contract Dollar Value; 6: Name of Procuring Organization; 7. Address of Procuring Organizations; 8. Procuring Organization Contracting Officer’s Name and Phone Number; 9. Procuring Organization Technical or Program Manager’s Name and Phone Number; 10. Indicate whether the contract was a prime contract or whether the contract was a subcontract to a prime contract. If a prime contract, insert ‘Prime contractor, if a subcontract, insert subcontractor. 11. Provide information on problems encountered on the identified contract and the corrective actions taken. PHASE II The most highly qualified firms will be selected and will be asked to participate in Phase II of this project. The firms selected for this phase will be provided minimum requirements and specifications for this project and requested to develop and submit conceptual plans and drawings and pricing for final evaluation. Design Freedom Requirements: Requirements stated in the request for proposal will be minimums. Innovative, creative, or cost-saving proposals that meet of exceed these requirements are encouraged and will receive quality points accordingly. The project must be designed and built with the funds available. PRICE EVALUATION FACTORS: Offerors are advised that an award may be made without discussion, exchanges or any contact concerning the proposals received; therefore, Offerors should present their best and final price at the time of submission. Offerors should not assume that they will be contacted or afforded an opportunity to qualify, discuss, or revise their proposals. Proposals will be evaluated on lump sum price for the base item that is most advantageous to the Government. Option items will be evaluated as to reasonableness of price and may be added at any time during the term of the contract as funds become available. The Government reserves the right to reject any or all proposals at any time prior to award; to negotiate with any or all Offerors on any prices, to award a contract to other than the offeror submitting the lowest total price; to award to the offeror submitting the proposal determined by the Government to be the most advantageous to the Government. The final selection will be determined based on Phase I proposal ranking and both the final technical proposal and the cost proposal submitted by each firm. Factors are equal in weight. SET-ASIDE NOTICE: This procurement has no restrictions and is full and open to all firms. MANDATORY REGISTRATION REQUIREMENT: FAR 52.204-7 Central Contractor Registration: Any and all firms or persons wishing to respond to this solicitation must be registered in the “Central Contractor Registration (CCR) database” which is the primary Government repository for Contractor information required for the conduct of business with the Government. Offerors and Contractors may obtain information on registration and annual confirmation requirements via the internet at http://www.ccr.gov or by calling 1-888-227-2423, or 269-961-5757. By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the CCR database prior to award, during performance, and through final payment of any contract. SPECIAL NOTES: Submittal requirements: One original with original signature and four copies ( 5 total) must be submitted. Submittal packages must be on 8 X 11 ˝ paper in neat orderly presentation folders with the name of the firm and project title on the cover page. Type size should be 12 Times New Roman for ease of reading. Unnecessary long proposal are discouraged and only pertent information is required. A PRE-PROPOSAL CONFERENCE WILL NOT BE HELD FOR PHASE I. IF INTERVIEWS ARE HELD THEY WILL BE AT A TIME AT A LOCATION ESTABLISHED BY THE GOVERNMENT. OFFERORS MUST FULLY RESPOND TO ALL THE REQUIRMENTS OF THIS SOLICITATION INCLUDING MANDATORY SUBMITTAL REQIRMENTS TO BE CONSIDERED. THERE ARE NO OTHER DOCUMENTS AVAILABE IN PHASE 1.
