Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E050044, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 624310 and the business size maximum is $5.0 Million. The proposed contract is set aside for small businesses. All responsible small business sources may submit a quote, which will be considered by the Government. This office is requesting quotes on the following item: 0001: Provide all travel, supplies, materials, and personnel to provide Counseling Services to the BIA, Keams Canyon Elementary School, Keams Canyon, AZ in accordance with the Scope of Work (SOW). Service shall be 7 hours per day for 3 days per week for 35 weeks. The QUANTITY is One (1), The UNIT is Lump Sum. Please provide a breakdown of your costs, which shall include at a minimum, hourly rate and travel costs (mileage and lodging). 0002: Provide all travel, supplies, materials, and personnel to provide Psycho-Educational Services to the BIA, Keams Canyon Elementary School, Keams Canyon, AZ in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. Please provide a breakdown of your costs, which shall include at a minimum, cost per evaluation (Initial and re-evaluations), hourly rate and travel costs (mileage and lodging). 0003: Provide all travel, supplies, materials, and personnel to provide Speech and Language Services to the BIA, Keams Canyon Elementary School, Keams Canyon, AZ in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. Please provide a breakdown of your costs, which shall include at a minimum, hourly rate and travel costs (mileage and lodging). Please provide a copy of your state certification for each discipline (i.e. counseling, psycho-education, and speech and language services). The Government intends to award a firm fixed price contract. The period of performance is from Date of Award (not later than August 25, 2005) through June 30, 2006. The SCOPE OF WORK (SOW) is as follows: Scope of Work. SCOPE. Contractor shall provide Counseling Services, Psycho-educational evaluations, and Speech and Language Services to students at Keams Canyon Elementary School (KCES), Keams, Canyon, Arizona. The contractor shall provide Counseling Services which shall include a providing a counselor. The counselor assigned will provide professional services that the administrator at KCES deems necessary for each child referred. This shall include individual or group counseling for students, consultation with parents, administrators and teachers, report writing, educational meeting as requested, and appropriate referral to Social Services agencies when needed. The contractor shall provide psycho-educational evaluations for special education students and referrals for the 2004-2005 school year. Services shall include: a. Comprehensive psycho-education evaluations including testing time with students, data collection, final report and the MET meeting. b. Comprehensive evaluations which includes extended measures (e.g. Emotional Disability/ADHD/Adaptive). c. Non-evaluation services such as specialized workshops staff training. Interns shall not be utilized for this service. The contractor shall provide speech and language services to include IEP development screenings, and comprehensive evaluations, speech & language trainings, and staff/parent consultations for identified students. Services shall also include child study team meetings, classroom observations, multi-disciplinary team meetings, provide therapy for each SLD student as stated in the IEP, and provide quarterly progress reports. The Offeror shall provide the lump sum cost for the base and four option year periods with a breakdown of their cost to provide this service and provide a copy of their certification. The offeror shall have a favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program prior to contract award (not later than August 22, 2005).
